摘要
目的探讨甲泼尼龙联合丙种球蛋白对病毒性脑炎患儿心理状况和预后的影响。方法将我院于2013年1月至2015年5月期间收治的94例病毒性脑炎患儿,随机分成实验组和对照组,每组47例。对照组使用阿昔洛韦及地塞米松常规等治疗,治疗组在常规治疗的基础上静脉增用甲泼尼龙和丙种球蛋白。使用父母用症状问卷(PSQ)、运动功能评分(FMA)和痉挛量表(ASS)评估两组患者治疗前后的心理及运动功能障碍,同时调查患者的不良反应。结果两组患者在治疗前的PSQ、FMA和ASS评分,组间对比差异不明显(P>0.05)。治疗后,实验组患者各项PSQ评分明显低于对照组(P<0.01),实验组和对照组的总有效率分别为91.11%、73.33%,差异有统计学意义(P<0.01),两组之间的不良反应发生率无统计学差异(P>0.05)。结论甲泼尼龙联合丙种球蛋白治疗有助于改善病毒性脑炎患儿的心理状况,且可行性、安全性良好。
Objective To explore effect of methylprednisolone combined with gamma globulin on psychological status and prognosis in the treatment of children with viral encephalitis. Methods 94 cases of children with viral encephalitis in our hospital from January 2013 to May 2015 were selected and randomly divided into the experimental group and the control group,47 cases for each.The control group were given conventional acyclovir and dexamethasone for treatment,while the experimental group on the basis of conventional treatment of vein increased therapy with methylprednisolone and gamma globulin. The psychological state and movement function before and after treatment were evaluated by PSQ,FMA and ASS. At the same time,the adverse reaction were surveyed as well. Results Before therapy,there were no statistical differences of the scores of PSQ、FMA and ASS between the two groups（P〉0.05）.After treatment,each single factor of PSQ of experimental group was lower than that of control group（P〈0.01）,the total effective rates of experimental group and control group were 91.11% and 73.33% respectively,and the difference reached the statistical level（P〈0.01）,there was no statistical difference in adverse reaction rate between the two groups（P〉0.05）. Conclusion Methylprednisolone combined with gamma globulin has better effect in improving psychological status in treatment of children with viral encephalitis children,and has good feasibility and security.
