摘要
目的:完善苏州市立医院(以下简称"我院")抗菌药物处方专项点评工作,促进临床合理用药。方法:实施持续质量改进措施,在处方点评工作中不断发现问题,结合医院实际情况,制定解决问题的方案,并进一步实施及调整。抽取2011年7月—2014年12月门、急诊抗菌药物处方共174 255张,对其进行专项点评。结果:实施持续质量改进措施后,我院门、急诊抗菌药物销售金额占药品总销售金额的比例由2011年的29.42%(12 089 092.29元/41 098 082.31元)逐步降至2014年的20.47%(11 203148.72元/54 740 644.66元);用药频度由2011年的847 075.23逐步降至2014年的562 325.01;门、急诊抗菌药物处方不合理率最高的为2011年12月的10.53%(503张/4 775张),后逐年降低,2014年各月处方不合理率均保持在1%以内。结论:我院已形成一套自己的持续质量改进工作程序,提高了我院抗菌药物合理应用水平,减少了抗菌药物的使用,但还需进一步完善。
OBJECTIVE：To improve the work of special comments on antibiotics prescriptions in Suzhou Municipal Hospital （ hereinafter referred to as “our hospital”） , and to promote the rational drug use in clinic .METHODS：Continuous quality improvement was conduced to find out the problems in antibiotics prescriptions .Combined with the actual situation of our hospital , the related scheme was formulated to solve these problems , and then the further implement and adjustment were adopted .Special comments were conducted on 174 255 outpatient and emergency antibiotics prescriptions which were extracted from Jul .2011 to Dec.2012.RESULTS： After the continuous quality improvement , the ratio of consumption sum of outpatient and emergency antibiotics versus the total consumption sum decreased from 29.42%（12 089 092.29 yuan/41 098 082.31 yuan） in 2011 to 20.47%（11 203 148.72 yuan/54 740 644.66 yuan）;and the defined daily dose system decreased from 847 075.23 in 2011 to 562 325.01 in 2014.The highest rate of irrational outpatient and emergency antibiotics prescriptions was 10.53%（ 503 prescriptions/4 775 prescriptions ） in Dec .2011 , which was in a decrease trend year by year , and maintained a low level within 1% in each month of 2014. CONCLUSIONS： Our hospital has formed the complete routine of continuous quality improvement , which increase the rational level of application of antibiotics , yet decrease the application amount of antibiotics .It still needs the further improvement .
