摘要
目的探讨新型超滤装置治疗难治性心力衰竭患者的有效性和安全性。方法2009年11月至2014年5月新疆医科大学附属中医医院心脏重症监护病房共收治急性失代偿心力衰竭(ADHF)患者329例,按人选标准和排除标准进行筛查,最终共人选难治性心力衰竭患者52例。观察超滤治疗前后人选患者的体重、呼吸困难评分、血氧饱和度和左心室射血分数(LVEF)、肾功能、电解质、血气分析等指标和主要不良事件的发生情况。结果(1)超滤治疗的有效性观察结果:入选的52例患者中23例经肘正中静脉、29例经股静脉建立体外循环通路。超滤治疗持续8~22h,总超滤量为(4489±1548)ml(1944~7613ml)。超滤治疗后患者平均体重由(75.4±8.7)kg减低至(69.8±8.4)kg(P〈0.01),呼吸困难评分、血氧饱和度及LVEF均明显改善(P均〈0.01)。(2)超滤治疗的安全性观察结果:超滤治疗期间和超滤治疗后72h内未发生死亡、心肌梗死、心力衰竭加重等严重不良事件,治疗期间未发生有治疗意义的低血压事件。2例患者股静脉穿刺处渗血,未予特殊处理自愈,1例滤器内凝血,更换滤器后继续治疗。超滤治疗前后患者血电解质、肾功能、酸碱平衡、血红蛋白和血小板计数未见明显变化。结论新型超滤装置可有效缓解难治性心力衰竭患者的液体潴留和呼吸困难,未见严重不良事件发生,疗效安全、可靠。
Objective To evaluate the efficacy and safety of a new ultrafihration device for treating refractory heart failure patients. Methods A total of 52 patients (37 male, age 29 - 85 ( 33 ± 44) years) with refractory heart failure were treated using a new ultrafihration device (FQ-16). Body weight, dyspnea score, oxygen saturation (SatO2), left ventricular ejection fraction (LVEF), BUN, creatinine, electrolytes and blood gas analysis were assessed before and after the treatment. Hypotension event and other main adverse events were recorded. Results Uhrafiltration duration ranged between 8 -22 hours. Total ultrafiltration volume was (4 489±1 548) ml. Compared with baseline, patients' body weight decreased from (75.3 ±8.74) kg to ( 69. 8 ± 8.39 ) kg ( P 〈 0. 01 ), dyspnea score improved from 2.47± 1.55 to 12. 87±3.61 (P 〈 0. 01 ) and SatO~ increased from 91.0 ± 6. 01 to 96.4±2. 52 ( P 〈 0. 01 ) and LVEF increased from ( 30.0± 4. 1 ) % to ( 36. 0 ±4. 3 ) % ( P 〈 0. 01 ) after ultrafiltration. Blood creatinine, BUN, electrolytes and blood gas analysis values were similar at baseline and post ultrafiltration. No hypotension event and other main adverse events occurred during the ultrafihration treatment. Conclusions The novel uhrafihration device adequately relieved hypervolemia and dyspnea in patients with refractory heart failure and the treatment process is safe in this patient cohort.
