摘要
目的比较苦参素联合窄谱中波紫外线(NB-UVB)在寻常型白癜风患者治疗前后血清炎症细胞因子水平变化,为白癜风的治疗提供新方向。方法将108例寻常型白癜风患者随机分为实验组和对照组,每组各54例,每组根据病情分期再分为稳定期及进展期各27例。实验组采用苦参素胶囊口服20 mg,3次/日,并配合NB-UVB照射治疗,3次/周;对照组仅采用NB-UVB照射治疗。两组疗程均为2月。结果复色情况比较:治疗2月后进展期患者实验组有效率为81.5%(22/27)高于对照组的55.6%(15/27),差异有统计学意义(x2=4.207,P=0.040);稳定期患者实验组有效率为70.4%(19/27)高于对照组的37.0%(10/27),差异有统计学意义(x2=6.033,P=0.014)。IL-6、IL-8及IL-17含量检测:治疗后实验组进展期与稳定期患者、对照组进展期患者血清中IL-6、IL-8、IL-17水平较治疗前降低(P<0.05);对照组稳定期患者IL-8、IL-17水平较治疗前降低(P<0.05),而IL-6水平变化不明显(P>0.05);实验组进展期与稳定期患者治疗后IL-6、IL-8、IL-17表达水平较对照组均下降,差异有统计学意义(P<0.05)。结论苦参素联合NB-UVB可显著下调寻常型白癜风患者体内IL-6、IL-8及IL-17的含量,促进皮损色素恢复。
Objective To compare the change of the levels of serum inflammatory cytokine of ordinary type of vitiligo patients before and after treatment of Oxymatrine combined with NB- UVB,thus,provide a new direction for the treatment of vitiligo. Methods 108 patients with vulgaris vitiligo were randomly assigned into experimental ground( 54) and control group( 54),and each group again was divided into stability and progress respectively with 27 patients according to the condition of their disease. The patients in the experimental group were treated with NB- UVB phototherapy combined with Oxymatrine capsules. Oxymatrine capsules were given 20 mg each time,three times a day. And those in the control group received NB- UVB treatment only. Both group had a treatment lasting for 2 month. Results 1. Repigment: After two months of treatment,the effective rates in progress patients were 81. 5%( 22 /27) in experimental group,which was higher than that of control group 55. 6%( 15 /27),the difference was statistically significant( x2= 4. 207,P= 0. 040); in stability group,the effective rate in experimental group was 70. 4%( 19 /27),which was also much higher than that of 37. 0%( 10 /27) in control group with a significant statistical difference( x2=6. 033,P = 0. 014); 2. Serum IL- 6,IL- 8,IL- 17 level: Serum IL- 6,IL- 8,IL- 17 level decreased in experimental group( progress &stable) and control group( progress)( P 〈 0. 05),but in control group( stable),the level of serum IL- 6 had no obvious change( P 〉 0. 05) in spite of the lower level of IL- 7,IL- 8( P 〈 0. 05). Compared with control group,both progress and stability vitiligo in therapy group reduced after treatment,the difference were statistically significant( P 〈 0. 05). Conclusion Oxymatrine combined with NB- UVB treatment could significantly lower the level of IL- 6,IL- 8,IL- 17 in serum,and promote repigment of skin lesions.
