摘要
目的 探讨医生应用“病情评估表”与护士应用“Barthel指数评定表”对患者分级护理级别的差异.方法 以210例本科住院患者为研究对象,由管床医师按照“病情评估表”进行年龄、体温、心率、呼吸、血压、血糖、氧饱和度、神志的观测,并评分.按评分标准将患者病情分为轻、中、重、危四组,予以下达三级、二级、一级及特级护理医嘱.同时护理人员对研究对象按照“Barthel指数评定表”予以评分,并根据标准判断患者为重度依赖、中度依赖、轻度依赖完和无需依赖护理四组.结果 分别对医师下达的特级护理、一级护理、二级护理及三级护理与护理人员判定的重度依赖、中度依赖、轻度依赖和无需依赖护理四组病例进行对比,Z=-0.339,差异无统计学意义（P＞0.05）.结论 应用“病情评估表”可将患者护理级别分值化,使医护人员对分级护理的认识更充分,与“Barthel指数评定表”相结合可使患者得到更适当的护理,是一个值得推广的方法.
Objective To explore the difference of the level about the grading nursing care between the doctor applicant disease assessment scale and nurse applicant Barthel index.Methods 210 patients hospitalized into our department were selected as study objects.Physicians observed and scored the age,body temperature,heart rate,breathing,blood pressure,blood glucose,oxygen saturation,and consciousness of the objects in accordance with disease assessment scale.According to the scoring criteria,the patients were divided into mild,moderate,severe,and critically severe four groups and were given three,two,one and special classes nursing advice correpondingly.At the same time,in accordance with the Barthl index evaluation scale,nursing staff scored and judged the patients with severely dependency,moderate dependency,mild dependency,and no need of dependency,and the patients were divided into four groups.Results There were no statistical differences in the judgment of physicians and nurses between these four groups （P 〉 0.05）.Conclusions The application of disease assessment scale can grade the patients＆#39; nursing level and improve the medical staff＆#39;s awareness of grading nursing care more fully.Combining Barthel index evaluation scale can more appropriately care the patients.It is a method worth being geralized.
