改良外侧横形入路可塑性跟骨钛板治疗Sanders Ⅱ~Ⅳ型跟骨骨折的疗效观察 被引量:8

摘要 目的 评价改良外侧横形入路可塑性跟骨钛板治疗SandersⅡ~Ⅳ型跟骨骨折的临床效果.方法 2012年7月至2015年7月行改良外侧横形入路与经典扩大外侧L形入路可塑性跟骨钛板治疗SandersⅡ~Ⅳ型跟骨骨折各34例,对比分析.结果 68例(70足)均获得随访,随访时间为6~18个月,平均12.5个月.治疗组无1例发生延迟愈合及不愈合,对照组术后3例(3足)出现切口边缘皮肤坏死、切口感染,2例(2足)出现腓肠神经损伤及腓骨肌腱不稳.两组术后皮瓣坏死发生率及切口感染率比较,差异有统计学意义(P<0.05).结论 改良外侧横形入路可塑性跟骨钛板治疗SandersⅡ~Ⅳ型跟骨骨折效果显著,有效预防了术后皮瓣坏死、切口感染、腓肠神经损伤及腓骨肌腱不稳等并发症. Objective To evaluate the clinical effect of improved lateral transverse approach of plastic calcaneus titanium plate for calcaneal fractures of Sandens type Ⅱ-Ⅳ.Methods July,2012 to July,2015,34 patients with calcaneal fractures of Sandens type Ⅱ-Ⅳ were treated with improved lateral transverse approach of plastic calcaneus titanium plate and 34 patients with calcaneal fractures of Sandens type Ⅱ-Ⅳ were treated with classical expanded lateral L-shaped approach of plastic calcaneus titanium plate.The treatment effects were compared.Results All the 68 patients (70 feet) were followed up for 6-18 months (average 12.5 month)s.No case got delayed union or nonunion in the treatment group.After the operation,3 cases (3 feet) got necrosis in the skin of incision edge and incision infection and 2 cases (2 feet) unstable sural nerve damage and peroneal tendons in the control group.There were statistical differences in the incidences of skin flap necrosis and incision infection between these two groups (P〈0.05).Conclusions Improved lateral transverse approach of plastic calcaneus titanium plate for calcaneal fractures of Sandens type Ⅱ-Ⅳ is effective and can effectively prevent postoperative complications,such as skin flap necrosis,incision infection,sural nerve damage,unstable peroneal tendons,etc.
