摘要
目的:比较分析分级诊疗制度下的新疆北疆的乌鲁木齐市、南疆的喀什市和东疆的哈密市三城市基层社区卫生机构2011—2015年资源配置与利用公平性情况。方法:通过绘制洛伦兹曲线、计算基尼系数来衡量基层社区卫生资源配置与利用的公平性。结果:新疆三城市社区卫生机构卫生技术人员配置欠公平,结构不合理、基本医疗利用不公平,难以完成分级诊疗下沉到社区卫生机构的任务;基本公共卫生服务利用较公平,发展趋势较好。结论:优化城市社区卫生资源配置,提高基本医疗利用水平,努力实现与分级诊疗工作的对接,实现保基本、保健康目标。
Objective： To compare and analyze the resource allocation and utilization of primary community health institutions in North, South and East of Xinjiang under hierarchy diagnosis system from 2011 to 2015. Methods： The allocation and ultilzation equity of primary community health resources were estimated by mapping Lorenz curve and calculating Gini coefficient. Results： There was inequity about health technical personnel configuration, unreasonable structure, inequity of basic medical utilization and could not finish community health institution task under hierarchy diagnosis in the three basic urban community system of Xinjiang. The uhilization of basic public health service was relatively fair and the development trend was well. Conclusion： It needed to optimize the urban health resource allocation, promote basic medical utilization level, make efforl to achieve matching of hierarchy diagnosis and achieve the goals for keeping base and health.
