3.0T磁共振成像扩散加权成像联合动态增强扫描对乳腺癌检查的技术探讨 被引量:9

摘要 目的探讨3.0 T磁共振成像(MRI)扩散加权成像(DWI)联合动态增强扫描在乳腺癌检查中的技术要点及图像质量保证的因素。方法收集本院进行乳腺MRI检查的80例女性患者行3.0T MR加DWI及动态增强扫描,T2-tirm-tra,T1-fl3d-tra-nonfs-radial,tirrm-right-sg,tirrm-left-sag,平面回波扩散加权成像(EPI-DWI),b值分别采用300,600,800 s/mm^2,及3D-FSPGR-FS-CE T_1WI动态增强扫描,分析各扫描序列及成像技术对图像质量的影响。结果 80例患者图像质量均达到诊断要求。T_1WI,及T_2WI脂肪抑制序列平扫及DWI可显示病灶形态、轮廓、大小、乳头有无凹陷,动态增强3D-FSPGR-FS-CE T_1WI可清楚显示病灶强化方式,内部血供方式,及病灶伴随症状,皮肤增厚,水肿,淋巴结肿大,胸大肌受累,血肿。结论扫描序列的正确选择,可提高成像的时间分辨率,减少运动伪影,运用脂肪抑制技术,DWI时选取恰当的b值,确保图像质量,提高乳腺癌诊断的精确性。 Objective To investigate the main technical points and factors of image quality of 3.0T MR diffu-sion weighted imaging in combination with dynamic enhanced scan in the examination of breast cancer. Methods Eighty cases underwent 3.0T MR DWI and dynamic enhanced scaning, T2-tirm-tra, T1-fl3d-tra-nonfs-radial,tirrm-right-sg,tirrm-left-sag, EPI-DWI, with b value of respectively 300, 600,800 s/mm2, and 3D-FSPGR-FS-CE T1WI dynamic enhanced scaning,then analyze the influence of the sequences and imaging techniques on image quality. Results The image quality of 80 cases reached the diagnostic requirements. By T1WI, T2WI and fat suppression sequence scan and DWI, the shape of the lesion, profile, the size , the nipple with and without depression could be showed,while by dy-namic enhanced 3D-FSPGR-FS-CE T1WI. The clear lesions, internal blood supply, lesions with symptoms, could be strengthen,such as skin thickening, edema, lymph nodes enlargement, pectoralis major muscle involvement, hematoma. Conclusion Right choice of scan sequence, can improve the imaging time resolution and reduce the mo-tion artifact. Using fat suppression, and selecting appropriate b value by DWI, doing help for ensuring the quality of image and improving the diagnostic accuracy of breast cancer.
