摘要
目的通过检测重度阻塞性睡眠呼吸暂停低通气综合征(OSAHS)合并代谢综合征(MS)患者手术治疗前后呼吸紊乱指数(AHI)、最低动脉血氧饱和度(Sa O2)、空腹血糖、血脂、超敏C反应蛋白(hs-CRP)、脂联素指标的改变,分析手术治疗重度OSAHS合并MS的疗效。方法搜集2013年3月至2015年3月在我科住院的重度OSAHS患者75例,合并MS的患者51例,均行上气道多平面手术治疗,术后3个月、6个月、1年进行随访,随访性能指标:AHI、最低Sa O2、空腹血糖、血脂、hs-CRP、脂联素。比较手术治疗前后上述指标的变化。结果术后3个月有效率为68.6%,术后6个月有效率为86.3%,术后1年有效率为88.2%。术后3个月随访,AHI、最低Sa O2、空腹血糖、血脂、hs-CRP、脂联素较术前差异有统计学意义;术后6个月随访,AHI、最低Sa O2、空腹血糖、血脂、hs-CRP、脂联素较术前差异有明显统计学意义;术后1年随访,AHI、最低Sa O2、空腹血糖、血脂、hs-CRP、脂联素较术后6个月差异无统计学意义。结论手术对OSAHS合并MS的患者治疗有明显的疗效,并明显降低患者血清hs-CRP水平,提高脂联素水平,改善患者预后。
Objective To analyze the efficacy of operative treatment in patients with OSAHS and MS as a combined dis- ease by examining the changes in the AHI, the lowest SaO 2, fasting blood-glucose, blood fat, hs-CRP and adiponectin before and af- ter operations. Methods We collected 75 sever OSAHS patients （51 of them were combined with MS）who were admitted in our hospital from March 2013 to March 2015 and received upper airway multi-platform operations. We followed patients up after 3 months ,6 months, 1 year of the treatment. The follow-up indexes included AHI, the lowest SaO2, fasting blood-glucose, blood fat, hs-CRP and adiponectin. We compared the indexes before and after operative treatment. Results The effective rate of the opera- tive treatment was 68.6% ,86. 3% and 88.2% after 3 months,6 months and 1 year of the operation respectively. The differences between the indexes （the AHI, the lowest SaO 2, fasting blood-glucose, blood fat, hs-CRP and adiponectin）measured before and after 3 months and 6 months of the operation were significant. The differences after 1 year of the operation were not significant. Conclu- sion In this study, operative treatment was associated with enhancement in curative effect, an significant decrease in serous hs-CRP,an increase in adiponectin and improvement in prognosis in patients with OSAHS as well as MS.
