摘要
目的探究跟骨板联合同种异体骨植骨治疗跟骨骨折的疗效。方法选取2012年1月至2014年1月延安大学附属医院收治的跟骨骨折患者98例,按照随机数字表法分为观察组(50例)和对照组(48例)。对照组患者采用跟骨钛板内固定术治疗,观察组患者采用跟骨钛板内固定联合同种异体骨植骨治疗。两组患者随访1年,观察两组患者治疗前后足部功能恢复以及术后并发症发生情况。结果观察组患者优良率高于对照组[92.0%(46/50)比66.7%(32/48)],差异有统计学意义(P<0.05)。对照组患者治疗后3、6、9、12个月的Maryland评分分别为(47±5)分、(64±4)分、(76±9)分、(86±10)分,观察组患者治疗后分别为(51±6)分、(72±7)分、(85±11)分、(91±12)分,两组患者组间、不同时点间、组间·不同时点间交互作用比较差异有统计学意义(P<0.05)。观察组切口愈合不良和负重疼痛发生率低于对照组[33.0%(16/50)比52.1%(25/48),10.0%(5/50)比25.0%(12/48)],差异有统计学意义(P<0.05)。结论跟骨板联合同种异体骨植骨治疗跟骨骨折效果良好,术后并发症发生率低,能明显改善患者Maryland评分,值得在临床推广。
Objective To investigate and analyze the effect of calcaneal plate combined with allogeneic bone graft in the treatment of calcaneal fractures. Methods Total of 98 patients with calcaneal fracture in Affiliated Hospital of Yan＇an University from Jan. 2012 to Jan. 2014 were included in the study ,and divided into an observation group and a control group according to the random number table method,50 cases and 48 cases respectively. The control group was treated with titanium plate internal fixation, while the observation group was treated with titanium plate internal fixation combined with allogeneic hone graft therapy. The patients were followed up for one year, and the functional recovery of foot and the complications after treat- ment in the two groups were observed. Results Excellent rate of the observation group was higher than the control group [ 92. 0% （ 46/50 ） vs 66.7% （ 32/48 ） ], the difference was statistically significant （ P 〈 0. 05 ）. After 3,6,9,12 months of treatment Maryland scores of the control group were （ 47 ± 5 ） scores, （64 ± 4 ） scores, （76 ± 9 ） scores, （ 86 ± 10 ） scores, of the observation group were （ 51 ± 6 ） scores, （72 ± 7 ） scores, （85 ± 11 ） scores, （91 ± 12 ） scores, the differences between the groups, between different time points and groups ± different time points was statistically significant （ P 〈 0. 05 ）. Poor cut and weight pain incidence of the observation group were lower than the control group[33.0% （16/50） vs 52. 1% （25/48）,10. 0% （ 5/50 ） vs 25.0% （ 12/48 ） ], the difference there was statistically significant （ P 〈0. 05 ）. Conclusion Calcaneal plate combined with allogeneic bone graft for calcaneal fractures has good effect, low incidence of postoperative complications, which can significantly improve the Maryland score, thus is worthy of the clinical promotion.
