摘要
目的:推广高才达名老中医药专家治疗肺胀经验。观察苓桂术甘汤合补阳还五汤治疗肺源性心脏病合并心功能不全的临床疗效。方法:将84例患者随机分为试验组和对照组各42例,对照组予:噻托溴铵喷雾剂,1吸/d;单硝酸异山梨酯20 mg,2次/d;口服氢氯噻氢25 mg,1次/d,螺内酯片40 mg,1次/d;试验组在对照组的基础上,予以口服苓桂术甘汤合补阳还五汤,水煎服,每次服100 m L,2次/d。2组疗程均为1个月。将2组治疗前后临床症状改善情况,B型氨基端利钠肽原(NT-pro-BNP),肺功能进行比较。结果:治疗后试验组临床疗效总有效率为72.5%,对照组为43.6%,试验组临床疗效总有效率高于对照组(P<0.05)。治疗后试验组FEV1、FEV1%、FEV1/FVC步行距离较对照组升高明显(P<0.01)。治疗后试验组NT-pro-BNP较对照组明显下降(P<0.01)。结论:苓桂术甘汤合补阳还五汤治疗慢性肺源性心脏病合并心功能不全患者疗效肯定,能有效改善患者临床症状,保护心肺功能,提高患者生活质量。高才达主任医师治疗肺胀的临床经验值得推广。
Abstract Objective： To publicize famous doctor Gao Caida＇ s experience in treating lung distention and to observe the clinical ef- ficacy of Linggui Zhugan Decoction combined with Buyang Huanwu Decoction in treating patients with chronic pulmonary heart dis- ease with cardiac insufficiency. Methods ： Eighty-four patients were chosen from shunyi district of Beijing Chinese medicine hospital from May 2013 to Jan. 2015 and randomly divided into two groups, namely the treatment group and the control group with 42 cases in each group. Patients in the control group were treated with tiotropium bromide nasal spray once a day, 20mg isosorbide dinitrate for two times a day, orally-taking 25 mg hydrogen chloride hydrogen hydroehlorothiazide once a day, and 40rag spironolactone tab- lets once a day, while patients in the treatment group were given Linggui Zhugan Decoction combined with Buyang Huanwu Decoc- tion twice a day with 100ml decoction solution each time. Results：The total effective rate of the treatment group was 72.5 %, while that of the control group was 43.6%. There was statistically significant difference in the total effective of the two groups （P 〈 0. 05）. FEV1, FEVI% , FEV1/FVC walking distance of the treatment group was significantly higher than those the control group after treatment （ P 〈 0. 01 ）. NT-pro-BNP of experimental group the treatment group significantly decreased compared with that of the control group after treatment （P 〈 0. 01 ）. Conclusion：The effectiveness of Linggui Zhugan Decoction combined with Buyang Huanwu Decoction on treating chronic pulmonary heart disease with cardiac insufficiency is obvious. It may relieve symptoms, pro- tect cardiac and pulmonary functions and improve the quality of life, which is worth of clinical promotion and application.
