摘要
目的观察灯盏细辛穴位注射联合浮针激光治疗带状疱疹神经痛的临床疗效。方法 412例带状疱疹神经痛患者按随机数字表法分为基础治疗组、浮针激光组、穴位注射组、联合组,各103例。基础治疗组基础治疗以静滴注射用阿昔洛韦抗病毒治疗,口服布洛芬止痛;另出疹部位及疼痛局部照射红光。穴位注射组予基础治疗,另灯盏细辛注射曲池、阳明泉、太冲穴、内关、足三里主穴,并辅以阿是穴。浮针激光组予基础治疗,另浮针激光痛点治疗。联合组予基础治疗,灯盏细辛注射液穴位注射,并对痛点进行浮针激光治疗。于出院前静脉采检测血清白细胞介素-2(IL-2)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)及前列腺素E2(PGE2)水平,统计患者皮疹结痂时间、红肿消退时间、神经痛持续时间、显效时间、治愈时间、神经痛复发率、总有效率等以比较临床疗效。结果与基础治疗组、浮针激光组及穴位注射组比较,观察组IL-2、IL-6、IL-8、PGE2及神经痛复发率明显降低,神经痛持续时间及治愈时间明显缩短,总有效率明显提高(均P<0.05)。结论灯盏细辛具有散寒解表及活血舒筋之功效,可抑制炎症因子刺激,减轻痛感,浮针激光可改善局部血液循环、调节组织生理机能及新陈代谢,两者联合治疗带状疱疹神经痛疗效确切。
Objective： To observe the clinical effect of acupoint injection of Erigeron breviscapus combined with floating needle laser on the treatment of herpes zoster neuralgia,and to study the mechanism providing theoretical basis for clinical treatment. Methods： 412 cases with herpes zoster neuralgia were randomly divided into the treatment group,floating needle laser group,acupoint injection group,and the observation group. They were treated with different methods. Before discharge,the following data was collected serum interleukin（IL-2 levels of IL-2,IL-6,IL-8 and prostaglandin E2（PGE2）,the statistics of the rash scab,swelling subsided time,neuralgia continuous time,effective time,cure time,neuralgia recurrence rate and the total efficiency to compare the clinical efficacy. Results： Compared with floating needle laser,acupoint injection group and the treatment group,IL-2,IL-6,IL-8,PGE2 and neuralgia recurrence rate of the observation group were significantly lower;neuralgic persistent time and healing time was significantly shorter;total efficiency was improved obviously（P〈0.05）. The difference was statistically significant. Conclusion： Erigeron breviscapus has the effects of dispelling cold,relieving,relaxing muscles and promoting blood circulation,inhibiting inflammatory cytokines and lessen the pain,while floating laser acupuncture can improve local blood circulation,regulating physiological function and metabolism. The combination therapy has an exactly curative effect,worthy to be popularized.
