微型钛板坚固内固定治疗下颌骨骨折的疗效观察 被引量:3

摘要 目的:观察分析对下颌骨骨折的患者采用微型钛板坚固内固定技术进行治疗的临床价值。方法:对口腔科收治的下颌骨骨折患者的临床资料进行回顾性分析研究,73例采用微型钛板坚固内固定技术进行治疗(观察组),87例采用拉力螺钉坚固内固定技术进行治疗(对照组)。术后随访3个月,对比观察两组患者临床疗效、咬合关系、颞颌关节功能、张口度、平均住院时间、疼痛程度及并发症等情况。结果:术后随访3个月,复查X线片显示所有患者的骨折对位良好,骨折全部愈合。观察组治疗有效率为98.63%,对照组为90.80%,两组之间差异有统计学意义(P=0.032)。观察组咬合关系矫正率100%,对照组矫正率97.70%,观察组的咬合关系优于对照组(P=0.192)。观察组颞颌关节功能恢复情况良好者72例(98.63%),对照组颞颌关节功能恢复情况良好者79例(90.80%),观察组的张口度和颞颌关节功能恢复情况优于对照组,差异有统计学意义(P<0.05)。观察组不良反应发生率为1.37%,对照组为6.90%,差异无统计学意义(P=0.089)。结论:采用微型钛板坚固内固定技术对下颌骨骨折的患者进行治疗,对位准确、稳定性强,可促进骨断端愈合,尽快恢复患者的局部解剖结构和咬合能力,是一种较理想的治疗方法。 Objective To observe and analyze the in patients with mandibular fracture with titanium miniplate rigid internal fixation for treatment of clinical value. Method The clinical data of the patients who were admitted to department of stomatology of mandibular fractures was retrospectively analysis,73 cases with titanium mini plate rigid internal fixation for treatment( observation group) and 87 cases with lag screw rigid internal fixation technique in the treatment( control group). Patients were followed up for3 months were observed and compared the clinical curative effect of two groups of patients,occluding relation,temporal mandibular joint function,mouth opening,average hospitalization time,the degree of pain and complications etc. Results Postoperative follow-up of 3 months,X- ray showed fractures of all patients were good,fracture healing. In the observation group,the effective rate was98. 63%,90. 80% in the control group,the difference between the two groups had statistical significance( P = 0. 032). The observation group occlusal correction rate was 100%,the control group correction rate was 97. 70%,the observation group of occlusion was better than that of control group( P = 0. 192). Observation group of temporomandibular joint function recovery in good condition 72cases( 98. 63%),the control group of temporomandibular joint function recovered well in 79 cases( 90. 80%),the observation group of opening and temporal mandibular joint function recovery was better than that of the control group,the difference was statistically significant( P〈0. 05). The adverse reaction rate of 1. 37%,6. 90% in the control group,the difference was not statistically significant( P = 0. 089). Conclusion The use of titanium mini plate rigid internal fixation for mandibular fracture patients for treatment,accurate positioning,strong stability,promote bone stump healing,as soon as possible to restore patients to the local anatomic structure and bite capacity,is an ideal method of treatment.
