摘要
目的探讨孕期体重合理增长数对孕产妇、胎儿及新生儿的影响。方法选择2013年10月至2014年12月于北京市平谷区医院进行孕检并分娩的孕妇274例,将患者按随机数字表法分为两组。研究组137例以孕前身体质量指数(BMI)资料进行分析后进行理想增长体重数的制定,包括合理的膳食及锻炼,使孕妇的体重增长在孕期正常体重范围内。对照组137例进行传统的健康教育讲座,未进行膳食及锻炼的干预,顺其自然增加。探究合理体重的增加对两组间胎儿、孕产妇、新生儿的临床影响。结果研究组体重增长在标准范围者占96.35%,对照组体重增长在标准范围者占46.72%,研究组显著高于对照组(χ2=82.87,P<0.05)。研究组妊娠合并糖尿病、妊娠期高血压、巨大儿、剖宫产、产后大出血发生率分别为2.19%、4.38%、3.65%、18.25%、2.19%,对照组妊娠合并糖尿病、妊娠期高血压、巨大儿、剖宫产、产后大出血发生率分别为7.30%、10.95%、11.68%、38.69%、8.03%,研究组显著低于对照组(χ2值分别为3.95、4.17、6.24、14.05、4.81,均P<0.05)。结论通过合理膳食、有效锻炼以及改善生活方式,使孕妇孕期体重得到了有效控制,降低了分娩后并发症的发生。
Objective To explore the effects of appropriate weight gain during pregnancy on mother, fetus and neonates. Methods A total of 274 cases of pregnant women were selected as research objects. All these women received pregnancy examinations and delivered their babies at Beijing Pinggu Hospital during the period of October 2013 to December 2014. They were divided into two groups in accordance with random number tables. As for all 137 cases of pregnant women in study group, materials concerning their body mass index (BMI) before pregnancy were analyzed, based on which an ideal objective for weight gain was set for them. ColTesponding interventions included rational diet and exercise so as to ensure their weight gain falling into a normal range. As for control group, all those 137 pregnant women received conventional lectures on health and didn' t receive any intervention of rational diet or exercise. Their weight increased in the absence of any weight management. Clinical effects of appropriate weight gain during pregnancy on fetuses, mothers and neonates in both groups were explored. Results As for the proportion of pregnant women whose weight gain fell into the normal range, the number reached 96.35% in the study group and 46.72% in the control group (Xz = 82.87, P 〈 0.05 ). The occurrence rate of gestational diabetes mellitus, gestational hypertension, fetal macrosomia, cesarean section, and postpartum hemorrhage amounted to 2. 19%, 4. 38%, 3.65%, 18.25% and 2.19%, respectively in the study group, and 7.30%, 10.95%, 11.68%, 38.69% and 8.03%, respectively in the control group. Thus, the occurrence rate was significant lower in the study group than that in the control group (X: value was 3.95, 4.17, 6.24, 14.05 and 4.81, respectively, all P 〈 0.05 ). Conclusion Interventions such as rational diet, effective exercise and lifestyle changes help to bring pregnant women' s weight under control, which in the end helps to reduce the incidence of complications after delivery.
