摘要
目的研究国际妇产科联合会(FIGO)Ⅰ-Ⅱ期的宫颈癌患者住院期间生存质量及其影响因素。方法采用横断面调查法,使用自制基本情况调查表和中文版宫颈癌生存质量测定量表(FACT-Cx)对2014年6月至2015年6月在陕西省肿瘤医院妇瘤科住院的201例FIGOⅠ-Ⅱ期宫颈癌患者进行问卷调查。单因素分析宫颈癌患者生存质量影响因素根据变量类型使用t检验或方差分析;多因素分析宫颈癌患者生存质量影响因素使用多重线性逐步回归分析并拟合最优方程。结果 1FACT-Cx量表总分(104.06±21.584);共性模块FACT-G总分(63.89±17.189);五个维度分数:生理状况得分(18.09±6.158);社会/家庭状况得分(19.63±4.988);情感状况得分(16.03±6.791);功能状况得分(10.13±5.071);附加关注得分(40.17±6.371);2单因素分析结果显示:不同工作情况(F=4.094,P=0.008)、家庭月收入(F=2.528,P=0.030)、家庭关系评价(F=14.072,P<0.001)、经济负担(F=12.680,P<0.001)之间的生存质量总分差异有统计学意义;3多重线性回归分析结果显示:对FIGOⅠ-Ⅱ期宫颈癌患者生存质量总分影响作用由大到小的因素依次为:经济负担、家庭关系评价、工作情况、医疗保险。结论 FIGOⅠ-Ⅱ期宫颈癌患者住院期间的生存质量水平不容乐观,尤其是功能领域处于中等偏下水平;对于经济负担重、家庭支持少、个人无稳定收入、无医疗保险的宫颈癌患者,值得医护人员加强关注,提供针对性地指导和帮助以提高其住院期间的生存质量。
Objective To investigate the quality of life （QOL） in patients with FIGO I- 11 stage cervical cancer（CC） during their hospitalization and its influencing factors. Methods Cross-sectional study was adopted and a questionnaire survey was carried out by using self-designed basic condition questionnaire and Chinese version of Functional Assessment of Cancer Therapy-Cervix （FACT-Cx）. A total of 201 cases of patients with FIGO I -II stage cervical cancer were surveyed. All these patients were hospitalized at the department of gynecological tumor in Shaanxi Provincial Tumor Hospital from June 2014 to June 2015. By using t-test or variance analysis in accordance with the types of variables, univariate analysis of the QOL in cervical cancer patients and its influencing factors was conducted. Multiple linear stepwise regression analysis was performed and the best-fit equation was obtained to realize the multivariate analysis of the influencing factors of QOL in CC patients. Results Aggregate score of FACT-Cx and FACT-G was 104. 06 ± 21. 584 and 63. 89 ± 17. 189, respectively. Among five measurements in FACT-G, the score of physical well-being reached 18.09 ± 6. 158, social/family well-being 19.63 ±4. 988, emotional well-being 16.03 ± 6. 791, functional well-being 10. 13 ± 5. 071 and additional attention 40. 17 ± 6. 371. Results of univariate analysis indicated that there were statistically significant differences in aggregate score of QOL among patients of different working conditions （ F = 4. 094, P = 0. 008 ）, monthly household income （ F = 2. 528, P = 0. 030）, family relationship （ F = 14. 072, P 〈 0. 001 ） and financial burden （ F = 12. 680, P 〈 0. 001 ）. Multiple linear regression analysis found that influencing factors of the aggregate score of QOL in patients with FIGO Ⅰ-Ⅱ stage cervical cancer in descending order were financial burden, family relationship, working conditions and medical insurance.
