摘要
目的：探讨胰十二指肠切除患者术后发生胰瘘的相关危险因素,为有效预防胰瘘的发生提供理论依据。方法：回顾性选择2012年1月~2015年1月期间河南省洛阳新区人民医院收治的100例已行胰十二指肠切除术的患者,并调查收集可能导致胰瘘的相关危险因素进行单因素及Logistic多因素分析。结果：本研究100例胰十二指肠切除术患者共16例发生胰瘘,总发生率为16%（16/100）。单因素分析显示上腹部手术史、术前胆红素、胰腺质地、胰管直径、术中出血量、术后血红蛋白、术后生长抑素使用是胰瘘发生的危险因素（均P〈0.05）。而多因素分析显示上腹部手术史、胰腺质地软、胰管直径细、术后血红蛋白低是胰十二指肠切除术后胰瘘发生的独立危险因素（OR=4.162、6.104、5.613、4.034,均P〈0.05）。结论：胰十二指肠切除术后胰瘘的发生与上腹部手术史、胰腺质地软、胰管直径细、术后血红蛋白低密切相关,故临床应根据患者情况采取有效措施以减少胰瘘的发生。
Objective：To explore the related risk factors for pancreatic fistula after pancreaticoduodenectomy to provide a theoretical evidence for effectively preventing the occurrence of pancreatic fistula.Methods：A total of 100 patients who were admitted in our hospital from January,2012 to January,201 5 and had performed pancreaticoduodenectomy were included in the study.The related risk factors for developing pancreatic fistula were collected for single factor and Logistic multi-factor analy-sis.Results：Among the included patients,1 6 had pancreatic fistula,and the total occurrence rate was 1 6% （1 6/100）.The sin-gle-factor analysis showed that the upper abdominal operation history,preoperative bilirubin,pancreatic texture,pancreatic duct diameter,intraoperative amount of bleeding,postoperative hemoglobin,and application of somatostatin after operation were the risk factors for developing pancreatic fistula （P 〈0.05 ）.The multi-factor analysis showed that the upper abdominal operation history,the soft pancreatic texture,small pancreatic duct diameter,and low postoperative hemoglobin were the de-pendent risk factors for developing pancreatic fistula （OR =4.1 62,6.104,5.613,4.034,P 〈0.05 ）.Conclusions：The occur-rence of pancreatic fistula after pancreaticoduodenectomy is closely associated with the upper abdominal operation history,the soft pancreatic texture,small pancreatic duct diameter,and low postoperative hemoglobin;therefore,effective measures should be taken to reduce the occurrence of pancreatic fistula according to the patients’own conditions.
