摘要
目的观察ⅠB2-ⅡA2期宫颈癌患者联合术前放化疗的近期疗效。方法将90例ⅠB2-ⅡA2期宫颈癌患者分为3组,分别进行术前的新辅助化疗、放疗、放化疗联合治疗,比较3组患者在治疗有效率、临床症状缓解情况以及发生副反应情况方面的差别。结果ⅠB2期患者和ⅡA1期患者的总有效率相当,ⅡA2期患者的总治疗有效率较ⅠB2期患者和ⅡA1期患者均降低,且差异具有统计学意义（χ2=10.743,P〈0.05;χ2=11.426,P〈0.05）。化疗组患者和放化疗组患者在不同临床分期患者间的总体治疗有效率差异无统计学意义（χ2=2.743,P〉0.05）,放疗组患者的总体治疗有效率较化疗组和放化疗组患者均较低,且差异具有统计学意义（χ2=9.256,P〈0.05;χ2=12.084,P〈0.05）。放化疗组患者和化疗组患者中非巨块型肿瘤的疗效略好于巨块型肿瘤患者,但差异不具有统计学意义;而放疗组中巨块型肿瘤患者的治疗有效率较非巨块型肿瘤患者好,且差异具有统计学意义（χ2=9.847,P〈0.05）。从总体有效率看,巨块型肿瘤患者和非巨块型肿瘤患者的疗效相差不显著（χ2=4.173,P〉0.05）。3种治疗方式对患者阴道出血、阴道排液的临床症状都能显著缓解。结论ⅠB2-ⅡA2期宫颈癌患者在术前进行放化疗联合辅助治疗,能有效改善临床症状,尤其对ⅠB2分期和ⅡA1分期的患者具有更好的治疗效果。
Objective To study the short-term efficacy of preoperative chemoradiotherapy for ⅠB2-ⅡA2 cervical cancer. Methods 90 patients were divided into 3 groups. The patients in chemo group were preoperatively treated by chemotherapy,the patients in radio group were preoperatively treated by radiotherapy,and the patients in radio-chemo group were preoperatively treated by chemoradiotherapy. The curative effects were observed. Results The total effective rates of stage ⅠB2 and stage ⅡA1patients were equivalent. The radio groups showed significantly high curative effects to the patient with giant cancer. The patients with ⅠB2 and ⅡA1 cervical cancer showed better curative effect than the patients with ⅡA2 cervical cancer. The clinical symptoms were significantly relieved. The total treatment effective rate of stage ⅡA2 patients decreased more than ⅠB2 patients andⅡA1 patients,the difference had statistical significance（ χ-2= 10. 743,P〈0. 05; χ-2= 11. 426,P〈0. 05）. The effective rate in radiotherapy group were lower than chemotherapy group and chemoradiotherapy group,the difference had statistical significance（ χ-2= 9. 256,P〈0. 05; χ-2= 12. 084,P〈0. 05）. The effective rate of bulky tumor patients in the radiation group was better than non bulky tumor patients,and the difference had statistical significance（ χ-2= 9. 847,P〈0. 05）. From the overall efficiency,the effect of bulky tumor patients and non bulky tumor patients had not significantly difference（ χ-2= 4. 173,P〈0. 05）. 3 types of treatment can significantly ease vaginal bleeding,vaginal discharge. Conclusion Preoperative chemoradiotherapy can improve clinical symptom of stage ⅠB2-ⅡA2 patients,and it has special effect for stage ⅠB2 and stage ⅡA1 patients.
