摘要
目的观察通脉养脑方联合西医常规治疗脑动脉硬化症的临床疗效。方法将98例脑动脉硬化症患者随机分为2组。对照组46例予常规西医治疗,治疗组52例在对照组治疗基础上加用通脉养脑方治疗。2组均治疗3个月统计疗效,并观察2组治疗前后血脂[胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)及高密度脂蛋白胆固醇(HDL-C)],血液流变学(全血黏度、血浆黏度、红细胞比容及纤维蛋白原),大脑中动脉(MCA)、椎动脉(VA)的收缩期流速(Vs)、血管搏动指数(PI)及阻力指数(RI)情况。结果治疗组疗效优于对照组(P<0.05)。2组治疗后TC、TG、LDL-C均降低(P<0.05),HDL-C升高(P<0.05),且治疗组改善优于对照组(P<0.05)。治疗组治疗后全血黏度、血浆黏度、红细胞比容及纤维蛋白原均较本组治疗前降低(P<0.05),而对照组除血浆黏度外,其余指标均降低(P<0.05),且治疗组治疗后全血黏度、血浆黏度、红细胞比容及纤维蛋白原低于对照组治疗后(P<0.05)。2组治疗后MCA、VA的Vs及RI、PI指标均较本组治疗前降低(P<0.05),且治疗组降低优于对照组(P<0.05)。结论通脉养脑方联合西医常规治疗脑动脉硬化症疗效确切,且能改善患者血脂、血液流变学及血流动力学指标。
Objective To observe the clinical effects of Tongmai- yangnao formula combined with routine western therapy on cerebral arteriosclerosis. Methods 98 subjects with cerebral arteriosclerosis were randomly divided into two groups. 46 subjects in control group were treated by routine western treatment. 52 subjects in treatment group were treated by Tongmai- yangnao formula on the basis of control group treatment. The curative effect was evaluated 3 months after treatment in two groups. The total cholesterol（ TC）,triglyceride（ TG）,low density lipoprotein cholesterol（ LDL- C）,high density lipoprotein cholesterol（ HDL- C）,whole blood viscosity,plasma viscosity（ SV）,hematocrit value（ HCT） and fibrinogen before and after treatment were observed in two groups,as well as the systolic velocity（ VS）,pulsatility and resistance indexes（ PI,RI） of the middle cerebral artery（ MCA） and vertebral artery（ VA）. Results The curative effect in treatment group was superior to that in control group（ P〈0. 05）.The levels of TC,TG,LDL- C were decreased after treatment in two groups（ P〈0. 05）,and the HDL- C was increased（ P〈0. 05）,the improvement in treatment group was superior to that in control group（ P〈0. 05）. The whole blood viscosity,SV,HCT and fibrinogen after treatment was decreased in treatment groups（ P〈0. 05）. The indexes in control group were decreased except the SV（ P〈0. 05）,and the decrease in treatment group was lower than that in control group（ P〈0. 05）. The Vs,RI and PI of MCA and VA after treatment were decreased in two groups（ P〈0. 05）,and the decrease in treatment group was superior to that in control group（ P〈0. 05）. Conclusion Tongmai- yangnao formula combined with routine western therapy has exact effect on the treatment of cerebral arteriosclerosis,can improve the indexes of blood lipid,hemorheology and hemodynamics.
