摘要
目的探讨中药益气养阴通脉汤预防老年髋部骨折术后下肢深静脉血栓(deep venous thrombosi DVT)形成的疗效及其安全性。方法将接受髋部骨折手术的符合研究标准的112例老年患者随机分为治疗组和对照组,治疗组55例术后给予益气养阴通脉汤治疗,对照组57例术后给予低分子肝素钙注射液治疗,对2组患者的患肢肿胀、D-二聚体、引流量及术后14d下肢彩色超声多普勒检查指标变化进行比较和分析。结果两组患肢周径差(患肢与健肢于膝上及膝下10cm测量周径的差值),差异无明显统计学意义(P>0.05)。术后第5天及第10天的D-二聚体变化差异有统计学意义(P<0.05)。术后10天治疗组中有2例发生了下肢深静脉血栓,发生率为3.36%;对照组中有3例发生了下肢深静脉血栓,发生率为5.26%;两组比较差异无统计意义(P>0.05)。结论中药益气养阴通脉汤对于老年髋部骨折术后下肢深静脉血栓形成具有良好的预防作用,与低分子肝素钙比较,其疗效相当,但相对不良作用更小,值得进一步推广使用。
Objective To investigate the effect and safety of Chinese medicine Yiqi Yangyin Tongmai decoction in the prevention of postoperative deep venous thrombosis（ DVT） of elderly hip fracture. Method 112 cases of elderly patients met the study criteria were randomly divided into treatment group and control group. The treatment group of 55 cases was given Yiqi Yangyin Tongmai decoction after surgery. The control group of 57 cases was given low molecular weight heparin injection therapy after surgery. Limb swelling,D- dimer,drainage and postoperative lower extremity 14 d color Doppler ultrasound of the two groups were compared and analyzed. Result The difference on two limb circumference between the two groups was not statistically significant（ P〈0. 05）. After 5 days and 10 days,the difference on D- dimer between the two groups was statistically significant（ P〈0. 05）. After 10 days,two cases in the treatment group had deep vein thrombosis occurred,and the incidence was 3. 36%. Three cases in the control group had deep vein thrombosis occurred,and the incidence rate was 5. 26%. The difference was not statistically significant（ P〉0.05）. Conclusion Chinese medicine Yiqi Yangyin Tongmai decoction had good preventive effect on DVT of elderly hip fracture. Comparing with low molecular weight heparin,it has similar curative effect,but the relative side effects are smaller,and it is worth to be popularized and used.
