不同类型脑卒中与血脂的关系及相互影响 被引量:10

Correlation between different types of cerebral apoplexy and blood lipids as well as the effects of blood lipids on different types of cerebral apoplexy

摘要 目的调查不同类型脑卒中与血脂的关系,以及血脂对不同类型卒中的影响差异。方法收集2011年1月至2015年1月神经内科与神经外科脑卒中住院患者的一般信息和血脂资料,患者均处于发病7 d内的急性期,总卒中组共1 882例,其中脑梗死组1 022例、脑出血组609例,蛛网膜下腔出血组251例,同时收集了1 495例同期无卒中史的门诊体检人员一般信息和血脂资料作为对照组。进一步将脑梗死组患者血脂资料与脑出血组进行比较,研究血脂对这两种卒中影响的异同。结果总卒中、脑梗死、脑出血患者平均血清总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)浓度与对照组比较,差异有统计学意义(P<0.05),脑梗死组患者与脑出血组差异有统计学意义(P<0.01),经多因素logistic分析,TC≥6.19 mmol/L时总卒中、脑梗死者的相关危险分别为1.788(P<0.01)、1.801(P<0.01),脑梗死对比脑出血的危险比为1.702(P<0.05);TG与总卒中、脑梗死及脑出血均无相关性,但TG≥2.27 mmol/L时,脑梗死对比脑出血的危险比为1.427(P<0.05);HDL-C<1.04 mmol/L者总卒中、脑梗死的相关危险分别为1.523(P<0.01)、1.801(P<0.01),脑梗死对比脑出血的危险比为1.566(P<0.01);LDL-C≥4.14 mmol/L者总卒中、脑出血的相关危险分别为0.644(P<0.01)、0.404(P=0.01)。结论血清TC、TG、HDL-C、LDL-C浓度与脑出血及蛛网膜下腔出血均无相关,血清TC与总卒中、脑梗死呈明显正相关,当TC≥6.19 mmol/L时发生脑梗死的风险高于脑出血;当TG≥2.27 mmol/L时发生脑梗死的风险高于脑出血;当HDL-C<1.04 mmol/L时发生脑梗死的风险高于脑出血;LDL-C与总卒中、脑出血呈明显负相关。 Objective To investigate the correlation between different types of cerebral apoplexy and blood lipids as well as the effects of blood lipids on different types of cerebral apoplexy. Methods The data including general information and blood fat levels of patients with cerebral apoplexy who were hospitalized in Department of Neurology and Department of Neurosurgery of our hospital from January 2011 to January 2015 were collected,all the patients were within 7 days of acute phase after development of disease in which there were 1882 cases of total stroke( total stroke group),1 022 cases of cerebral infarction( CI group),609 cases of intracranial hemorrhage( ICH group),251 cases of subarachnoid hemorrhage( SAH group),besides,1 495 healthy subjects were served as control group. The data of all the subjects were analyzed and compared among the 5 groups,and the effects of blood fat on different types of cerebral apoplexy were analyzed. Results There were significant differences in the serum levels of total cholesterol( TC),trigalloyl glycerol( TG),HDL-C,LDL-C between total stroke group,CI group,ICH group and control group( P〈0. 05),moreover,There were significant differences between CI group and ICH group( P〈0. 01). The multiple factor logistic analysis showed that in TC≥6. 19 mmol / L,the relative risk in stroke group and CI group was 1. 788,1. 801,respectively( P〈0. 01). The risk ratio of CI vs ICH was 1. 702( P〈0. 05).The serum levels of TG were not correlated to total stroke,CI,ICH,however,in TG≥2. 27 mmol / L,the risk ratio of CI vs ICH was 1. 427( P〈0. 05). In HDL-C 1. 04 mmol / L,the relative risk in total stroke group,CI group was 1. 523,1. 801,respectively( P〈0. 01),and the risk ratio of CI vs ICH was 1. 566( P〈0. 01). In LDL-C≥4. 14 mmol / L,the relative risk in total stroke group,ICH group was 0. 644,0. 404,respectively( P〈0. 05).
