摘要
目的探讨糖尿病周围神经病变(DPN)相关危险因素及早期筛查的临床意义。方法使用密歇根神经病变筛查量表(MNSI)进行门诊糖尿病患者DPN筛查,并分析MNSI分值与年龄、性别、病程、BMI、WHR、吸烟史、血压、血糖、血脂、HbA_1c及DR的关系。结果 MNSI量表检查,>4分且足部查体>2分患者占38.2%。单因素线性回归分析发现,病程[回归方程Y=4.552+0.125X(t=6.588,F=43.402,P<0.001,R=0.279)]、年龄[回归方程Y=4.064+0.04241X(t=8.440,F=71.230,P<0.001,R=0.677)]与MNSI分值呈正相关。WHR、BMI、HbA_1c、TC、TG、LDL-C、HDL-C、FPG、2 hPG、SBP、DBP、糖尿病家族史与MNSI分值无相关性(P>0.05)。组间秩和检验发现,伴DR者[6.5%(5%,10%)vs 5%(4%,7%)]及女性患者E5.5(4,7)vs 4.5(3,6)]MNSI分值较高。多元线性回归显示,MNSI分值与病程、年龄最相关,回归方程Y=1.936+0093X_1+0.046X_2,X_1=病程,X_2=年龄(F=9.176,P<0.001)。结论我院门诊DPN患病率为38.2%。病程、年龄、DR是DPN的危险因素。
Objective To investigate early screening for diabetic peripheral neuropathy（DPN） and its related risk factors.Methods DPN was screened in diabetes outpatients clinics by Michigan Neuropathy Screening Instrument（MNSI） scale.The relationship between age,sex,duration of diabetes,body mass index（BMI）,waist to hip rate（WHR）,blood pressure,plasma glucose,blood lipid HbA_1c diabetic retinopathy（DR） and MNSI score,were evaluated.Results There were 38.2%patients with MNSI questionnaire score higher than 4 and foot examination score higher than 2.Linear-regression analysis showed that MNSI score was positively correlated with duration of diabetes（Y=4.552＋0.125 X,t=6.588,F=43.402,P〈0.001,R=0.279） and age（Y=4.064＋0.04241 X,t=8.440,F=71.230,P〈0.001,R=0.6 7 7）.However,MNSI scores was not associated with WHR,BMI,HbA_1c,TC,TG,LDL-C,HDL-C,FPG,2 hPG,SBP,DBP and family history of diabetes.Rank sum test between groups showed that the MNSI scores were higher in patients with diabetic retinopathy and female patients[6.5%（5%,10%）vs 5%（4%,7%）][5.5（4,7） vs 4.5（3,6）]respectively.Multiple regression analysis showed that MNSI score was significantly positively correlated withduration of diabetes and age（regression equation：Y= 1.936＋0093 X_1＋0.046 X_2,X_1 was duration of diabetes,X_2 was age,F= 9.176,P〈0.001）.Conclusion The prevalence of DPN in Diabetic outpatient clinic is 38.2%.Age,duration of diabetes and DR are the risk factors for DPN.Age and duration of diabetes are the most influencing factors.
