摘要
目的探讨螺旋CT三期增强扫描在肝血管瘤诊断中的应用价值。方法选取2012年1月至2014年12月66例肝血管瘤患者进行研究，均行螺旋CT三期增强扫描，观察平扫、延时及动态增强扫描影像特征。结果66例肝血管瘤患者患者中单发病灶61例、多发病灶5例，病灶数目共71个，直径0．9～7．3cm。直径〈2cm49个（69．0％），≥2cm22个（31．0％）。平扫：低密度灶61个（85．9％）、等密度灶7个（9．9％）、高密度灶3个（4．2％）。增强扫描：60个为动脉期强化病灶，其中39个为边缘结节状、簇状强化，21个为片状不规则强化。10个病灶为门脉期，对比剂向病灶中心填充，病灶开始强化。延迟扫描：对比剂进一步向病灶中心填充，59个（83．1％）为等密度填充、10个（14．1％）呈不规则部分填充、2个（2．8％）中央部位存在裂隙状更低密度区。结论螺旋cT三期增强扫描可较好把握多期扫描时间以捕捉不同时相典型表现，为肝血管瘤的确诊提供有力依据，也可鉴别诊断肝血管瘤与原发性肝癌，临床应用价值高。
Objective To analyze agnosis of hepatic hemangioma. Methods the application value of spiral CT three-phase scan in the di- The 66 patients with hepatic hemangioma from January 2012 to December 2014 were studied and were all treated with spiral CT three-phase scan. The imaging char- acteristics of plain scanning, delayed and dynamic enhanced scan were observed. Results Among the 66 cases of patients with hepatic hemangioma, there were 61 cases with single lesion and 5 cases with multiple lesions with 71 lesions in total with diameter of 0. 9-7.3 cm. There were 49 lesions with diameter less than 2 cm（69% ）, 22 lesions of or more than 2 cm（31% ）. Plain scan： 61 low density lesions （85.9%）, 7 equidensite lesions（9.9% ） and 3 high density lesions（4.2% ）. Enhanced scan： 60 arte- rial phase enhancement lesions, including 39 peripheral nodular and tufted enhancement and 21 sheet ir- regular enhancement. There were 10 lesions in portal phase and contrast agents were filled to the center of the lesions and lesions began to be strengthened. Delayed scanning： contrast agents were further filled to the center of the lesions. There were 59（83.1% ） equidensite filling, 10（ 14. 1% ） showing irregular filling and 2 （2. 8% ） slit like lower density area in central part. Conclusions Spiral CT three-phase scan can better grasp the scanning time to capture the typical performance in different phases, provide a strong basis for the diagnosis of hepatic hemangioma and can differentially diagnoze hepatic hemangioma and hepatocellular carcinoma with high clinical application value.
