摘要
目的探讨中心静脉导管堵管后再通的时间窗。方法通过体外模拟中心静脉堵管分4h(A组及a组)、8h(B组及h组)、12h(C组及c组)、24h(D组及d组)、48h(E组及e组)10组用尿激酶1000U/ml进行负压溶栓,观察其效果。结果相同的堵管时间条件下,两种不同的静脉导管所需要的再通时间比较差异无统计学意义(P〉0.05)。PICC组中4h与8h、8h与12h之间比较差异具有统计学意义(P〈0.05),而12h组与24h间比较差异无统计学意义(P〉0.05)。CVC管中4h与8h之间比较差异无统计学意义(P〉0.05),8h与12h、12h与24h组间比较差异具有统计学意义(P〈0.05)。结论中心静脉导管堵管后再通成功率以及所需时间与堵管时间密切相关,中心静脉堵管发生回血返流应及时冲管.正压封管,发牛堵管后及时讲行再通的操作。
Objective To investigate the time-window of the recanalization of central venous catheter (CVC) occlusion. Methods CVC and PICC were divided into five groups respectively according to the different time of occlusion of central venous catheter in vitro, and explored the timing-effect of urokinase on recanalization by vacuum. Results There was statistically significant difference in the time of recanalization of PICC between group 4 h and group 8 h, group 8 h and group 12 h, no statistically significant difference in the time of recanalization between group 12 h and group 24 h. The results indicated the time needed on recanalization of PICC was positively correlated to the time of occlusion within 12 h. There was no statistically significant difference in the time of recanalization of CVC between group 4 h and group 8 h, there was statistically significant difference in the time of recanalization between group 8 h and group 12 h, group 12 h and group 24 h. The results indicated the time needed on recanalization of CVC was positively correlated to the time of occlusion beyond 8 h. Conclusions The success rate and the time needed ofrecanalization were closely related to the occlusion time. Central venous catheter should be sealed with positive pressure immediately after canalization. The recanalization of catheter should be operated in time in case of occlusion.
