摘要
目的:探讨胰管支架内引流术缓解伴有胰管扩张的中晚期胰腺癌患者腹痛的疗效及安全性.方法:选取2014-02/2015-12河北省开滦总医院收治的伴有胰管扩张的中晚期胰腺癌患者84例,应用随机数字表法分为治疗组与对照组,各42例.对照组患者给予胆管支架内引流术,治疗组患者给予胰管支架内引流术联合胆管支架内引流术,对比治疗前后两组患者的血清总胆红素(total bilirubin,T B I L)、血淀粉酶等指标水平的变化,及手术前后患者腹痛程度,并对比两组患者并发症发生情况.结果:治疗前,两组患者的各指标对比无显著差异(P>0.05);治疗后,治疗组与对照组患者的Tbil、ALP、ALT水平均明显下降,差异有统计学意义(P<0.05),但两组患者的血淀粉酶的水平均较治疗前无显著变化(P>0.05),且两组患者Tbil、ALP、ALT水平也无明显差异(P>0.05);手术前,治疗组与对照组患者的VAS评分无显著差异(P>0.05),手术后各时间段治疗组患者的VA S评分均明显较对照组低(P<0.05);治疗组并发症总发生率为19.05%,对照组总发生率为21.43%,差异无统计学意义(P>0.05).结论:胰管支架内引流术缓解伴有胰管扩张的中晚期胰腺癌患者腹痛的疗效显著,值得临床推广应用.
AIM：To investigate the efficacy and safety of internal drainage of pancreatic duct with a stent in the management of abdominal pain in patients with advanced pancreatic cancer associated with pancreatic duct dilatation.METHODS：From February 2014 to December2015,84 advanced pancreatic cancer patients with pancreatic duct dilation treated at Kailuan General Hospital were selected and divided into either a treatment group or a control group,with 42 cases in each group.The control group was treated by biliary stent drainage,and the treatment group was treated by pancreatic duct stent drainage combined with biliary stent drainage.Serum total bilirubin（TBIL）,serum amylase and degree of abdominal pain were compared between the two groups before and after treatment.Complications were also compared between the two groups.RESULTS：Before treatment,there was no significant difference in tested indexes between the two groups（P 0.05）;after treatment,Tbil,ALP and ALT were significantly decreased in both groups（P〈0.05）,although serum amylase had no significant change（P〈0.05）.There was no significant difference between the two groups in TBIL,ALP or ALT after treatment（P〈0.05）.Before treatment,VAS score had no significant difference between the two groups（P〉0.05）.After treatment,VAS scores at different time points were significantly lower in the treatment group than in the control group（P〈0.05）.The incidence of overall complications did not differ significantly between the two groups（19.05%vs 21.43%,P 0.05）.CONCLUSION：Internal drainage of pancreatic duct with a stent in the management of abdominal pain in patients with advanced pancreatic cancer with pancreatic duct dilatation is effective.
