摘要
目的探讨含血心脏停搏液在婴幼儿先天性心脏病(CHD)手术中的应用及其对患儿心肌的保护作用。方法采取数字随机法将深圳市儿童医院收治的64例CHD患儿分成甲乙两组各32例,于手术中分别给予其冷晶体停搏液(HTK液)与自体冷血4∶1心脏停搏液输注,观察两种停搏液对患儿心肌的保护作用。结果两组在术前及主动脉开放1h、6h、24h,检查心肌酶、心肌复跳时间无明显变化,差异不显著(P>0.05),两组在CK-MB与c Tn I血清浓度指标均有所增加,且差异显著(P<0.05)。结论在CHD患儿体外循环心内直视手术中,自体冷血停搏液的应用,能更好保存患儿心肌细胞能量,减少对心肌的损伤,发挥更好的心肌保护作用。
Objective To investigate the application of blood containing cardiac arrest fluid in infants and young children with congenital heart disease（ CHD） and its protective effect on myocardium. Methods 64 cases of CHD patients hospitalized in Shenzhen Children ＇s Hospital were divided into two groups,32 cases in each,in operation they were given the cold crystalloid cardioplegic solution（ HTK） and autologous blood 4： 1heart stop cardioplegia infusion,and protective effect of two kinds of cardioplegia solution on myocardial was observed. Results The myocardial enzyme,myocardial re- jump time,sinus heart rate time and low rejection rate in preoperative and at the time point 1h,6h and 24 h after aortic opening had no significant change,there was no significant difference（ P 0. 05）. CK- MB and c Tn I concentration indexes of the two groups were significant increased（ P 0. 05）. Conclusions The application of autologous blood cardioplegic solution in CHD patients undergoing cardiopulmonary bypass heart surgery,can preserve myocardial cell energy,reduce damage to the myocardial,play a better role in myocardial protection.
