摘要
目的构建基于透明的医院感染指标的选择标准体系,促进医院感染指标的公开透明,实现透明监管化。方法采用文献回顾法和专题小组讨论法拟定初始选择标准,然后甄选20名医院感染管理领域专家,通过两轮德尔菲（Delphi）专家咨询筛选和确定选择标准。结果共有19名专家有效回复了两轮函询,其中94.74%具有副高级以上职称,84.21%从事专业年限10年以上;第1轮和第2轮咨询的专家积极系数分别为100.00%和95.00%,肯德尔（Kendall）协调系数分别为0.201（χ^2=68.44,P〈0.001）和0.323（χ^2=67.48,P〈0.001）,专家权威系数平均为0.90;经两轮专家咨询后最终共确定9个选择标准：准确性、可预防性、数据的易获得性、影响性、数据的易管理性、及时性、易风险调整性、理解性和普适性,各指标需要性评分均〉4.0分,且变异系数均〈0.25。结论 Delphi法在构建选择标准体系时具有良好的效果,该研究为科学选择公开报告的医院感染指标提供了依据。
OBJECTIVE To construct a selection criteria system of hospital infection indicators based on transparency so as to promote the public reporting of hospital infection indicators and achieve the transparent supervision.METHODS The initial selection criteria indexes were drawn by means of the literature reviewing and panel discussion,then 20 experts who were engaged in the infection control were selected to participate in two-rounds of Delphi experts′consultation so as to screen and confirm the selection criteria indexes.RESULTS A total of 19 experts responded to the two rounds of consultation effectively,94.74% of them had sub-senior or senior professional titles,84.21% of them were engaged in the infection control for more than 10 years.The active coefficients of the first and second round were 100.00%and 95.00%respectively,the Kendall＇s coefficients were 0.201（χ^2=68.44,P〈0.001）and 0.323（χ^2=67.48,P〈0.001）,respectively,and the average authoritative coefficient was 0.90.Finally,nine selection criteria indexes were confirmed after two-rounds of experts′consultation,including accuracy,preventability,ease of data collection,impact,ease of data management,timeliness,appropriate risk adjustment,comprehension,and universality.The scoring of desirability of each indicator was greater than 4.0points,and the coefficients of variations were all less than 0.25.CONCLUSION Delphi method has a good effect on the construction of the selection criteria system,and this study provides a basis for the scientific selection of hospital infection indicators.
