双源CT冠状动脉成像意外检出肺动静脉畸形分析

Analysis of Unexpected Detection of Pulmonary Arteriovenous Malformation by Dual Source Coronary CT Angiography

摘要 目的总结双源CT冠状动脉成像(CCTA)意外检出肺动静脉畸形(PAVM)的特点。方法回顾性分析2011年8月至2015年5月临床怀疑冠心病行CCTA检查的38153例患者的临床资料,总结意外检出PAVM的部位、形态及大小。结果意外检出PAVM 8例,占0.21‰。共发现9个病灶,全部为单纯型,其中,病灶位于左肺上叶舌段3个,左肺下叶3个,右肺中叶2个,右肺下叶1个,且病灶多较小,直径5~22 mm。结论 CCTA上PAVM的意外检出率为0.21‰。患者临床症状不典型,CCTA检查冠状动脉未见有意义狭窄,扫描野内见肺静脉提前显影和/或畸形的肺血管时,影像医师应常规扩大扫描野评价是否存在肺血管病变。 Objective To summarize the characteristics of unexpected detection of pulmonary arteriovenous malformation( PAVM) by dualsource coronary CT angiography( CCTA),to improve the understanding of PAVM and provide reference for clinic. Methods A total of 38153 patients with the suspected coronary heart disease were analyzed retrospectively from August 2011 to May 2015 and summarized the position,shape and size of PAVM. Results There were 8 patients( 9 lesions) having the unexpected detection of PAVM,accounting for 0. 21‰( 8 /38153). All of them were simple type,and among them,3 cases were in lingual segment of upper lobe of the left lung,3 cases were in inferior lobe of the left lung,2 cases were in middle lobe of the right lung,1 case was in inferior lobe of the right lung. Most of PAVM were small and 5 ~ 22 mm in diameter. Conclusion Unexpected detection rate of PAVM by CCTA was 0. 21‰. Radiologists should routinely enlarge the scanning field and evaluate the pulmonary vascular lesions when patients without typical symptoms in clinic or significant coronary artery stenosis by CCTA,or with pulmonary vein enhancement in advance and(or) malformed pulmonary vascular.
