摘要
目的评价长方案IVF-ET过程中出现慢反应的患者使用尿源性高纯卵泡刺激素(HP-FSH)的临床结局。方法回顾性分析行长方案降调节后对卵泡刺激素(FSH)呈慢反应的患者687例,根据所使用药物的不同分为A组(重组FSH组)373例和B组(重组FSH联合尿源性FSH组)314例,比较两组患者的基础状态、临床指标及助孕结局。结果 A组患者促性腺激素(Gn)天数、Gn总量、FSH总量均低于B组,Gn费用、获卵数及因卵巢过度刺激综合征(OHSS)全胚冻存率高于B组,差异均有统计学意义(P<0.05);两组的FSH启动剂量、HMG总量、LH水平、移植胚胎数、可利用胚胎数比较差异均无统计学意义(P>0.05);两组的胚胎种植率、临床妊娠率、早期流产率差异均无统计学意义(P>0.05)。结论对于卵巢储备功能正常的慢反应患者,使用尿源性高纯FSH可以获得与重组FSH相似妊娠结局并降低促排卵药物的费用和OHSS的发生率。
Objective To evaluate the clinical outcomes of patients with slow response in the course of long protocol in vitro fertilization and embryo transfer (IVF-ET) treated by highly purified urinary follicle stimulating hormone (HP-FSH) . Methods A total of 687 patients with slow response to FSH after long protocol down-regulation were analyzed retrospectively, then they were divided into A group ( recombinant FSH group, 373 cases) and B group (recombinant FSH combined with HP-FSH group, 314 cases) according to different drugs. The basic situations, clinical parameters, and IVF outcomes in the two groups were compared. Results The duration time (day) of gonadotrophin (Gn), the total dose of Gn and FSH in A group were statistically significantly lower than those in B group (P〈0.05), the cost of Gn, the number of oocytes retrieved, and the rate of whole embryo cryopreservation due to ovarian hyperstimulation syndrome (OHSS) in A group were statistically significantly higher than those in B group (P〈0. 05) ; there was no statistically significant difference in start dose of FSH, the total dose of human menopausal gonadotropin (HMG), luteinizing hormone (LH) level, the number of embryos transferred, the number of available embryos, embryo implantation rate, clinical pregnancy rate, and early abortion rate between the two groups (P〉0. 05 ) . Conclusion For the slow response patients with normal ovarian reserve function, HP-FSH has similar pregnancy outcome as recombinant FSH, and HP-FSH can reduce the cost of Gn and incidence rate of OHSS.
