摘要
患者女,43岁。上腹部红色皮损伴发轻度瘙痒4月余。查体:上腹泛发红色斑疹、斑片、斑丘疹及丘疹,部分皮损呈拱形或半环形排列。血TRUST 1∶16(+),TPPA(+)。皮损组织病理示∶真皮大量淋巴细胞浸润,散在浆细胞和组织细胞浸润,部分淋巴细胞异型性改变,表皮少数淋巴细胞浸润。诊断为二期梅毒。苄星青霉素治疗后皮损显著好转。
A 43-year-old female presented with red rashes on abdomen with mild itching for more than 4 months. Phys- ical examination showed generalized erythematous macules, patches, maculopapules and papules on her up- per abdomen, part of lesions were arranged in arched or semi-circular pattern. Serological test revealed both TRUST （ 1 ： 16 ） and TPPA were positive. Histopathological examination showed a large number of lympho- cytes infiltrate with scattered plasma cells and histiocytes in the dermis. Some lymphocytes displayed atypia. In addition, a few lymphocytes infiltrate were observed in the epidermis. Therefore, the diagnosis of second- ary syphilis was established. The skin lesions were remarkably improved after treated with intramuscular in- jection of benzathine with three times.
