绝经后骨质疏松与外周血单核细胞培养上清细胞因子的关系 被引量:4

摘要 目的 观察绝经前后妇女外周血单核细胞培养上清肿瘤坏死因子 (TNFα) ,白细胞介素 1(IL - 1) ,白细胞介素 - 6 (IL - 6 )水平变化及其与骨质疏松的相关性。方法 应用单光子骨密度仪测量非优势前臂尺桡骨中远端 1 3交界处桡骨骨密度 (BMD) ,小于 0 6 0 1g cm2 ( x - 2s)则诊为骨质疏松 (OP) ,据此将 5 5名妇女分为绝经前正常妇女 (NOP1) ,绝经后非骨质疏松妇女 (NOP2 ) ,绝经后骨质疏松妇女 (OP) 3组 ,测定其外周血单核细胞 (PBMC)培养上清中TNFα、IL - 1、IL - 6水平及血清中其他代谢指标。结果 OP组TNFα水平明显高于NOP1及NOP2组 (P <0 0 5 ) ,NOP2组虽有升高但差异无显著性 (P =0 0 8) ;IL - 1、IL - 6在绝经后水平升高 (P <0 0 5 ) ,骨质疏松组升高更明显(P <0 0 1)。IL - 1、IL - 6、TNFα与年龄、绝经年限呈显著正相关 (P <0 0 1) ,与BMD、E2 呈显著负相关 (P <0 0 5 ) ,与Ca Cr、Hyp Cr密切正相关 (P <0 0 5 )。血清PTH在OP组明显高于NOP1组 (P <0 0 5 )及NOP2组 (P <0 0 1) ,NOP1,NOP2组间差异无显著性 (P >0 0 5 )。结论 绝经后雌激素水平的下降 ,年龄的增长 ,是启动细胞因子IL - 1、IL - 6、TNFα生成的主要因素 ,增加的细胞因子促进骨吸收增多 。 Objective\ To observe whether the cytokines(IL-1,IL-6,TNFα)of peripheral blood mononuclear cells(PBMC) are associated with postmenoposal primary osteoporosis. Methods\ BMD at the junction of the middle and the distal thirds of non dominant ulna was measured by single photon bone densitometer in 55 women.BMD<0 601 g/cm 2( -2s) serves as the diagnostic criterion for osteoporosis(OP).According to this,55 women were divided into three groups:premenopausal group(NOP1, n =17,mean age 48±3),normal postmenopausal group(NOP2, n =20,mean age 58±4),postmenopausal osteoporotic group(OP, n =18,mean age 60±5).The release of IL-1,6,TNFα from PBMC,and other markers of bone metabolism were determined. Results\ TNFα level was higher in OP group than in NOP1 and NOP2( P <0 05),with not significant difference( P =0 08).IL-1 and IL-6 were higher in postmenopausal women( P <0 05),and highest in OP( P <0 01).IL-1,IL-6,and TNFα had significantly positive correlation with age,menopause duration ( P <0 01),and with Ca/Cr,Hyp/Cr( P <0 05),and had significantly negative correlation with BMD and E 2( P <0 05).PTH in serum was higher in OP than in NOP1( P <0 05),and than in NOP2( P 〈0 01),but there was not significant difference between NOP1 and NOP2( P >0 05). Conclusion\ The decrease in E2 level after menopause and the aging is the main factors in promoting the secretion of IL-1,IL-6 and TNFα,the high level of cytokines activate osteoclasts,with the elapse of time resulting in osteoporosis.High level of serum PTH is one of risk factors of postmenopausal osteoporosis.\;
