摘要
目的 :观察肾脏病患者的听力状况 ,探讨肾脏病患者听力损伤的影响因素。方法 :应用纯音测听 ,听觉脑干诱发电位 (BAEP)、4 0 Hz听觉相关电位 (40 Hz- AERP)检测了 78例肾脏病患者 (15 6耳 )的听功能。应用美国贝克曼 CX11生化仪检测了上述病人的血肌酐、尿素氮、钾、钠、磷、血脂水平 ,同时检测血红蛋白浓度。结果 :1有 37.18%的肾脏病患者存在听功能障碍 ,主要表现为以高频受损为主的感音神经性聋 ;2肾功能不全组的 BAER和 4 0 Hz- AERP的异常程度大于肾功能正常组 ;3肾脏病患者 BAEP的异常与血肌酐、尿素氮呈正相关 ,与血红蛋白呈负相关 ,与血脂代谢紊乱、钾、钠、钙、磷代谢紊乱有关。 4 0 Hz- AERP异常与血红蛋白呈负相关 ,与血脂代谢紊乱、电解质紊乱有关。结论 :肾脏病患者存在有高频受损为主的感音神经性聋 。
Objective: To investigate the hearing levels of patients with nephrosis and its influential factors for nephrosis patients.Methods:Seventy eight patients of nephrosis were diagnosed and divided into two groups,41 cases were renal function normal (group A),37 cases abnormal (group B).The pure tone audiometry,brain stem auditory evoked potential (BAEP) and 40 Hz auditory event related potential (40Hz\|AERP)were examined for evaluation of the hearing function in all patients.The blood biochemical indexes were examined by Beckman CX11 Biochemiometer which included Creatinine (Cr),Urea Nitrogen (BUN),Kalium(K),Sodium(Na),Calcium (Ca),Phosphorus(P),Blood\|lipid and Hemoglobin(Hb) in 78 patients with nephrosis. Results:① The rate of hearing damage was 37 18% in 78 patients with nephrosis which showed sensorineural hearing loss most on high frequency.② The serious disfunctional hearing of BAEP and 40Hz\|AERP were showed in group B than that in group A.③ In patients of nephrosis,the hearing damage was related with uremic toxins,anemia,blood lipid metabolic disturbances and electrolyte imbalance.Conclusion:The examination of hearing function in patients with nephrosis showed that the hearing loss was most on high frequency damage which had clinical significance for evaluation of prognosis of nephrosis patients.
