摘要
目的 探讨单孔腹腔镜疝囊高位结扎术治疗小儿复发疝的临床效果。方法 选取2009年3月-2015年3月在本院接受再次治疗的20例小儿复发疝患者作为研究对象,随机分成观察组和对照组,各10例。观察组采用单孔腹腔镜疝囊高位结扎术,对照组采用小切口手术,比较两组的治疗效果。结果 观察组的手术时间为（17.6±2.4）min,显著短于对照组的（22.8±2.7）min,差异有统计学意义（P〈0.05）。观察组的术中出血量为（17.7±5.4）ml,显著少于对照组的（23.8±6.8）ml,差异有统计学意义（P〈0.05）。所有患儿麻醉清醒后6-8 h即可进食流质食物,术后1-2 d下地活动,术后2-4 d出院。观察组无一例复发,对照组术后发生1例伤口感染和1例血肿。结论 单孔腹腔镜疝囊高位结扎术在小儿复发疝手术中的应用效果显著,安全可靠,损伤小,手术时间短,值得临床推广应用。
Objective To explore the clinical effect of high ligation hernial sac under single-port laparoscope in the treatment of pediatric recurrent hernia. Methods 20 children with recurrent hernia treated again from March 2009 to March 2015 in our hospital were selected and randomly divided into the observation group and the control group,10 cases in each group.The observation group was treated with high ligation hernial sac under single-port laparoscope,the control group was treated with small incision.The therapeutic effect in the two groups was compared. Results The oper- ation time in the observation group was （17.6±2.4） min,which was shorter than （22.8±2.7） rain in the control group,with significant difference （P〈0.05）.The intraoperative amount of bleeding in the observation group was （17.7±5.4） ml,which was less than （23.8±6.8） ml in the control group,with significant difference （P〈0.05）.After awaking from anesthesia, liq- uid diet can be taken within 6 to 8 hours,off-bed for movement 1 to 2 days,and discharge from hospital 2 to 4 days af- ter surgery.In the observation group,there was no recurrence,while in the control group,one case with wound infection and one with hematoma after surgery were displayed. Conclusion The application effect of high ligation hernial sac un- der single-port laparoscope in the treatment of pediatric recurrent hernia is significant,which is safe and reliable on pe- diatric recurrent hernia at advantages of mild damage and short operation time,it is worthy of clinical promotion and ap- plication.
