摘要
目的探讨胱抑素C(Cys-C)、a_1-微球蛋白(a1-MG)、β_2-微球蛋白(β2-MG)等多项肾功能联检在高血压病早期肾损害诊断中的价值。方法回顾性分析111例高血压病患者根据24h尿白蛋白排泄率(UAER)的不同,分成2组,单纯高血压(A组)58例和高血压早期肾损害(B组)组53例以及55例健康体检者(C组)的血清样本在东芝TBA-2000FR全自动生化分析仪上进行多项肾功能指标检测结果,用Pearson相关性分析与肾功能传统评价指标的相关性、上述指标在三组间和多项指标联检A组、B组间的差异。结果 Cys-C、a_1-MG和β2-MG分别与SCre和SUrea呈正相关(r值分别为0.685,0.716,0.599,0.615,0.735,0.699,P<0.05)。B组检测Cys-C、a_1-MG和β2-MG水平均显著高于A组和C组,差异有高度统计学意义(P<0.01),其他指标三组间差异无统计学意义(P>0.05);B组中三项指标联检的阳性检出率(83.02%)明显高于任意单一指标的阳性检出率(49.06%,45.28%,37.74%)和A组联检(1.69%),比较差异有统计学意义(P<0.05),A组单项指标的阳性检出率与多项联检的阳性检出率比较,差异无统计学意义(P>0.05)。结论 Cys-C、a_1-MG和β_2-MG可作为高血压病早期肾损害的敏感检测指标,联检能较早地反映高血压早期肾损害的程度,为临床医生对其早期诊断、病情监测等方面提供有价值的实验室依据。
Objective Discussion of cystatin C（Cys-C）,a_1-microglobulin（a_1-MG）,β_2-microglobulin（β_2-MG）etc.a number of kidney function inspection in the diagnosis of early renal damage of hypertension.Methods Retrospective analysis of 111 cases of hypertension patients according to urinary albumin excretion rate（UAER）,divided into two groups,the simple hypertension（group A）58cases and early renal damage in essential hypertension（group B）group53and 55 healthy persons（Group C）serum samples in Toshiba TBA-2000 FR full automatic biochemical analyzer for renal function indicators of test results,correlation with Pearson correlation analysis and renal function of traditional evaluation index,the index in among the three groups and a number of indexes inspection in group A,group B between difference.Results Cys-C,a1-MG and β_2-MG and SCre and SUrea were positively correlated（r=0.685,0.716,0.599,0.6150.735,0.699,P0.05）;group B testing of Cys-C,a1-MG and β_2-MG level were significantly higher than group A and group C,the difference is statistically significant（P0.01）,the difference in other indexes among the three groups were no statistical significance（P 0.05）.In group B,three indexes inspection of positive testing rate（83.02%）was significantly higher than that of any single index positive rate（4 9.06%,45.28%,37.74%）and group a inspection（1.69%）,the difference has statistical significance（P0.05）,group a single index of positive testing rate and the number of inspection the positive testing rate,the difference was not statistically significant（P0.05）.Conclusion Cys-C,a_1-MG and β_2-MG can be used as early renal damage in hypertension sensitive testing index,joint inspection can early reflect the degree of early renal damage in patients with essential hypertension,provide valuable laboratory basis for clinicians in the early diagnosis and condition monitoring.
