摘要
金匮肾气丸中隐藏着张仲景的几大治肾原则,蕴含了《黄帝内经》"化不可代"思想,其组方思路可很好指导慢性肾功能不全的治疗,厘正了祛邪要服从于扶正的关系。慢性肾功能不全患者肾虚是五脏亏虚的基础,肾虚又以肾精亏虚为核心,肾气、肾阳、肾阴化生乏源,进一步引起痰湿、瘀血留滞肾络,具有因虚致实,虚气留滞的病机特点。金匮肾气丸组方思路启示慢性肾功能不全治疗应竣补肾精以治形质,补肾应结合涩补精气,佐以利湿化瘀以通补精气,结合清浮游相火以清补肾气。
There were some therapeutic principles of healing kidney which were hiding in Jinkui Shenqi Pills, it contained the idea of ‘qi activity can not be substituted' in Huangdi Neijing. It could guide the treatment of chronic renal insufficiency very well, which improved the relationship of ‘eliminating pathogenic factor should be subordinated to strengthening vital qi. Kidney deficiency was the basis of the deficiency of five zang. Kidney essence deficiency was the core of kidney deficiency. The metaplasia deficiency of kidney qi, kidney yin and kidney yang could lead to the phlegm and blood stasis left in the kidney collateral. The pathogenesis features were ‘excess resulted from deficiency' and ‘deficiency and qi stagnation'. The prescription thinking of Jinkui Shenqi Pills reminded that the treatment of chronic renal insufficiency should be nourishing kidney essence to treat the shape. Tonifying kidney should also be combined with tonifying essential qi and accompanied by promoting diuresis and removing blood stasis to tonify essential qi and clearing ministerial fire to tonify kidney qi.
