摘要
目的观察益气活血泻肺利水方对慢性心力衰竭患者血浆脑钠肽(BNP)及左室射血分数(LVEF)的影响。方法将80例慢性心力衰竭患者随机分为2组。对照组40例予吸氧、限钠、限水,绝对卧床,酌情服用利尿剂、血管紧张素转换酶抑制剂(ACEI)或血管紧张素Ⅱ受体阻滞剂和β受体阻滞剂,根据患者病情酌情选用洋地黄类强心药,治疗组40例在对照组治疗基础上加用益气活血泻肺利水方治疗。2组均2周为1个疗程,1个疗程后统计临床疗效,并观察2组治疗前后BNP、LVEF及中医证候评分情况。结果 2组总有效率比较差异无统计学意义(P>0.05),2组疗效相当。2组治疗后气短、疲倦乏力、水肿评分均降低(P<0.05),且治疗组降低优于对照组(P<0.05)。2组治疗后BNP均降低(P<0.05),且治疗组降低优于对照组(P<0.05);2组治疗后LVEF均升高(P<0.05),且治疗组升高优于对照组(P<0.05)。结论益气活血泻肺利水方可降低慢性心力衰竭患者血浆BNP,提高LVEF。
Objective To observe the effects of Yiqihuoxue- xiefeilishui formula on plasma brain natriuretic peptide（ BNP） and left ventricular ejection fraction（ LVEF） in patients with chronic cardiac failure（ CHF）. Methods 80 subjects with CHF were randomly divided into two groups. 40 subjects in control group were received oxygen uptake,sodium and water restriction,strict bed rest,and consider the diuretic,ACEI,ARB and beta- blockers,and the digitalis was also taken conditionally. 40 subjects in treatment group were treated by Yiqihuoxue- xiefeilishui formula on the basis of control group treatment. The course was two weeks in two groups. The curative effect was evaluated after a course. The LVEF,BNP and traditional Chinese medicine（ TCM） syndrome score before and after treatment were observed in two groups. Results There was no statistical difference on total effective rate between two groups（ P〉0. 05）. The syndrome scores of breathe hard,fatigued and edema after treatment were decreased in two groups（ P〈0. 05）,and the decrease in treatment group was superior to that in control group（ P〈0. 05）. The BNP after treatment was decreased in two groups（ P〈0. 05）,and the decrease in treatment group was superior to that in control group（ P〈0. 05）. The LVEF after treatment was increased in two groups（ P〈0. 05）,and the increase in treatment group was superior to that in control group（ P〈0. 05）. Conclusion Yiqihuoxue-xiefeilishui formula can decrease the plasma BNP in CHF patients,and enhance LVEF.
