摘要
目的应用实时剪切波弹性成像(SWE)技术测量甲状腺结节的弹性模量值,探讨其在甲状腺结节良恶性鉴别诊断中的价值。方法应用SWE技术测量84枚甲状腺结节的弹性模量值,以术后病理结果为金标准,绘制ROC曲线,分析鉴别甲状腺良恶性结节的参考界值。结果甲状腺良恶性结节弹性模量的平均值、最小值、最大值、取样框内弹性数据离散度值、Emean-p值及Emean-m值比较差异均有统计学意义(均P<0.05)。各项弹性模量值诊断甲状腺恶性结节的ROC曲线下面积分别为0.8922、0.7175、0.9125、0.8991、0.9470及0.9147,诊断界值分别为22.5 k Pa、14.3 k Pa、47.2 k Pa、8.3 k Pa、2.6及2.6。结论甲状腺结节的各项弹性模量值均有助于其良恶性的鉴别诊断,其中最大值对于鉴别良恶性最具价值。
Objective To assess the value of real-time shear wave elastography(SWE) in the differential diagnosis of benign and malignant thyroid nodules with the elastic modulus value. Methods A total of 84 nodules were enrolled in this study. The elasticity indices(EIs) were measured by real-time SWE,and ROC curve was made. Using the pathologic results as standard,the best cut-off value was set and used to differentiate thyroid nodules. Results All EIs of malignant thyroid nodules,including Emean,Emin,Emax,Esd,Emean-p and Emean-m were significantly higher than those of benign nodules(all P〈0.05). The area under ROC curve were 0.8922,0.7175,0.9125,0.8991,0.9470 and 0.9147,respectively,the optimal cut-off value of EIs for predicting malignancy were 22.5 kPa of Emean,14.3 kPa of Emin,47.2 kPa of Emax,8.3 kPa of Esd,2.6 of Emean-p,and 2.6 of Emean-m. Conclusion EIs are helpful in differentiating malignant and benign thyroid nodules. In all references used in this study,Emax is the most significant one.
