摘要 目的:了解住院患者尿培养中分离的病原菌分布及耐药状况,为尿液感染性疾病的诊断、治疗提供用药依据。方法:对有尿路感染症状尿液培养出细菌的229例患者进行尿液细菌培养鉴定及药敏实验,并对细菌的耐药进行统计分析。结果:尿路感染致病菌以革兰阴性杆菌(190株)为主(82.9%),分列前3位的是大肠埃希菌(72.6%)、铜绿假单胞菌(7.9%)、肺炎克雷伯菌(4.7%);革兰阳性球菌(39株)以粪肠球(31.7%)和屎肠球菌(46.2%)为主,革兰阴性杆菌对头孢哌酮/舒巴坦、哌拉西林舒巴坦、阿米卡星敏感性较高,而对左氧氟沙星、头孢曲松耐药性较高。革兰阳性球菌对万古霉素、替考拉宁、利奈唑胺敏感性较高,对红霉素、四环素、利福平耐药性较高。结论:革兰阴性杆菌为尿路感染的主要致病菌,治疗预防应选用酶抑制剂等耐药性低的药物,临床合理选用抗菌药具有重要意义。 Objective:To investigate distribution and resistance of pathogens in inpatients with urinary culture,to provide the reference for the clinical effective diagnosis and treatment of urinary tract infection.Methods:The bacteria identification and drug sensitivity of inpatients with urinary tract infection were tested and the data of drug resistance of bacteria were analyzed.Results:Pathogens in inpatients with urinary culture were mainly gram negative bacteria(190 strains,82.9%),the first 3 were Escherichia coli(72.6%),Pseudomonas aeruginosa(7.9%),Klebsiella pneumoniae(4.7%).Gram-positive coccus(39 strains)were Enterococcus faecalis(31.7%) and Enterococcus faecium(46.2%).Drug sensitivity of Cefoperazone/Shubatan, piperacillin sulbactam and amikacin were high for gram negative bacteria,but levofloxacin and ceftriaxone were low.Drug sensitivity of vancomycin,linezolid,teicoplanin were high for gram-positive coccus,but erythromycin, tetracycline, rifampin were low.Conclusions:Knowing the primary pathogens of urinary tract infection and enzyme inhibitor in the low resistance drug is of great significance to guide the clinical rational use of antimicrobial drugs.
