七氟醚对行非心脏手术冠心病患者的心肌保护作用 被引量:6

摘要 目的评价七氟醚麻醉对行非心脏手术的冠心病患者的心肌保护作用。方法非心脏手术冠心病患者62例,随机分为静脉麻醉对照组30例和七氟醚组32例,比较两组麻醉方式对围手术期心肌缺血事件和心功能的不同影响。结果与术前比较,术后24 h七氟醚组的各观察指标无显著变化;而对照组的CK、CK-Mb、BNP显著升高,LVEF显著降低,室壁/室间隔搏幅减低和心电图异常的比例均显著升高,与七氟醚组比较有统计学差异(P<0.05)。结论七氟醚麻醉可在冠心病患者接受非心脏手术时减少围术期心肌缺血事件及心力衰竭的发生,为冠心病患者接受非心脏手术时一种安全的麻醉方式。 Objective To evaluate the protective effects of sevoflurane anesthesia on the myocardium of patients with coronary heart disease after non-cardiac surgery. Methods Sixty-two patients with coronary artery disease taking non-cardiac surgery were randomly divided into control group(30 patients) and treatment group(32 patients). Comparison was made in the different effects between two anesthesia methods on the perioperative myocardial ischemia events and cardiac function in the two groups. Results Compared with the indicators before the operation, the observation indicators in the sevoflurane group 24 h after the surgery had no changes, while the CK, CK-Mb, and BNP in the control group significantly increased, and the LVEF decreased. The ventricular wall/septum pulsation amplitude decreased, and the proportion of electrocardiographic abnormality increased in the control group. There were significant difference in between the two groups(P 0.05). Conclusion Sevoflurane anesthesia can reduce the incidence of perioperative myocardial ischemic events and heart failure in patients with coronary heart diseases taking non-cardiac surgery. It is a safe anesthesia for those patients who are going to receive non-cardiac surgery.
