摘要
目的探讨阿托伐他汀对不同阶段血管性认知功能障碍(VCI)患者血脂和认知功能的影响。方法 156例VCI患者随机分为对照组76例和观察组80例。对照组无痴呆型血管认知功能障碍(VCIND)患者45例,血管性痴呆(VaD)患者31例;观察组VCIND患者47例,Va D患者33例。对照组给予常规治疗,观察组在对照组基础上加服阿托伐他汀。对比治疗前后2组血脂水平及认知功能状况。结果治疗后,观察组VCIND及Va D患者总胆固醇(TC)和低密度脂蛋白胆固醇(LDLC)水平显著低于治疗前及对照组(P<0.05或P<0.01),简易智能状态量表(MMSE)评分及蒙特利尔认知评估量表(Mo CA)评分均高于治疗前和对照组(P<0.01);观察组治疗前后VCIND患者MMSE及Mo CA评分差值显著高于VaD患者(P<0.01),而TC、LDL-C及HDL-C水平变化与Va D患者无显著差异(P>0.05)。结论早期应用阿托伐他汀可显著改善VCI患者血脂水平和认知功能,延缓VCI疾病进展。
Objective To investigate the effect of atorvastatin on blood lipids levels and cognitive function of patients with different stages of vascular cognitive impairment( VCI). Methods A total of 156 VCI patients were randomly divided into the control group [n = 76,with 45 cases of VCI without dementia( VCIND) and 31 case of vascular dementia( VaD) ] and the observation group( n = 80,with 47 cases of VCIND and 33 case of VaD). All patients received conventional treatment and the observation group was treated with atorvastatin additionally. The blood lipids levels and cognitive function before and after treatment were compared between two groups. Results The serum levels of total cholesterol( TC) and low density lipoprotein-cholesterol( LDL-C) in the observation after treatment were significantly lower than those before treatment and in the control group( P〈 0. 05 or P 0. 01),while the score of mini mental state estimate( MMSE) and montreal cognitive assessment( MoCA) in the observation after treatment were significantly higher than those before treatment and in the control group( P〈 0. 01). In observation group,the improvement of MMSE and MoCA in VCIND patients were significantly higher than those in VaD patients( P〈 0. 01),but there were no significant differences in changes of TC、LDL-C and high density lipoprotein-cholesterol( HDL-C) levels between VCIND and VaD patients( P〈 0. 05). Conclusion Early application of atorvastatin may improve the blood lipids levels and delay the progress of VCI.
