摘要
老年男性患者,临床表现肾病综合征,血清单克隆游离轻链比例异常,免疫固定电泳见κ型Ig G单克隆免疫球蛋白条带,肾活检病理提示肾淀粉样变性合并C3肾小球病,间质大量CD20+细胞浸润。骨髓穿刺及活检提示淋巴浆细胞高度增生。结合临床、病理最终诊断慢性B淋巴细胞白血病、肾淀粉样变性合并C3肾小球病。
A 68-year-old man was admitted for nephrotic syndrome. His serum free light chain ratio was abnormal,and immunofixation electrophoresis test was positive. His renal biopsy revealed amylodiosis with C3 deposition,renal interstitial infiltrating lymphocytes that were positive of CD20. Bone marrow puncture and biopsy revealed abnormal Bcell lymphoproliferation. The final diagnosis for the case was B-cell chronic lymphocytic leukemia（ B-CLL）,renal amyloidosis with C3 glomerulopathy.
