摘要
目的探究胎儿超声心动图检查在胎儿心律失常的诊断应用,并评价其对胎儿心脏结构异常的诊断价值。方法对我院接收的73例孕妇进行胎儿超声心动图检查结果及临床资料进行回顾性分析。观察胎儿心律失常与心脏结构异常的相关性,评价胎儿超声心动图应用价值。结果通过随访后发现,73例孕妇中,因胎儿心脏结构异常实施引产手术2例(2.74%),胎儿存活66例(90.41%),胎儿死亡率(6.85%);中晚期孕妇超声心动图显示,孕中期孕妇胎儿心律失常超声心动图检出机率由13.69%,减至10.96%,其中孕晚期妇女8例心律失常胎儿有6例通过对症治疗治愈,而2例由于心力衰竭并发水肿最终致死;5例孕晚期孕妇胎儿心脏结构异常中,心脏结构异常类型有法洛四联征、三尖瓣闭锁、完全心内膜缺损、单心房、室间隔缺损等,其中3例心脏结构异常胎儿合并有心律失常。孕中期孕妇胎儿超声心动图检测,心脏结构异常检出由9.58%,降至6.84%;其中2例引产、2例分娩存活,死亡1例。结论应用超声心动图检测中晚期妇女体内胎儿的心脏结构异常以及心律失常较为安全可靠,具有重要筛查与诊断的意义。
Objective:To explore diagnostic application of fetal echocardiography in fetal cardiac arrhythmia,and to evaluate its diagnostic value of abnormal fetal cardiac structures. Methods:Examination results and clinical data of seventy three pregnant women admitted by the Hospital undergoing fetal echocardiography were retrospectively analyzed. The correlation between fetal cardiac arrhythmias and abnormal cardiac structures were observed,and the application value of fetal echocardiography was evaluated. Results:It was found in follow-ups that 2 out of 73(2.74%)pregnant women were subject to induced labor surgeries because of abnormal fetal cardiac structures,66 cases(90.41%),66 fetuses survived,and the mortality of fetuses hit 6.85%;The echocardiography for middle-late pregnant women showed that the detection probability of echocardiography of pregnant women with fetal arrhythmia during second trimester decreased from 13.69% to 10.96%,6 arrhythmia fetuses out of 8 late-stage pregnant women were healed via symptomatic treatment,and eventually died due to heart failure concurrently with edema;cardiac structures of 2 fetuses out of 5 pregnant women were abnormal,types of cardiac structural abnormalities involved the tetralogy of Fallot,tricuspid atresia,complete endocardial defect and defects of single atrial and ventricular septal,3 fetuses with abnormal cardiac structures suffered from arrhythmia. In fetal echocardiography for pregnant women during pregnant metaphase,the detection rate of abnormal cardiac structures dropped from 9.58% to 6.84%,including 2 fetuses undergoing induced labor,2 fetuses having live births and one fetus dead. Conclusion:The application of echocardiography to detect middle-late women with abnormal fetal cardiac structures in vivo and arrhythmia is safe and reliable,delivering great screening and diagnosis significance.
