摘要
目的探讨无创正压通气治疗重度急性左心衰效果。方法重度急性左心衰患者50例,随机分为常规组与观察组,观察治疗前后心率、呼吸频率、动脉血气分析:氧分压、二氧化碳分压、血氧饱和度的变化,记录临床症状缓解时间。所获数据采用方差分析,t检验和2检验。结果两组治疗前后心率、呼吸频率、氧分压、二氧化碳分压、血氧饱和度均有明显改善,常规组症状缓解时间(83.4±8.6)min,观察组症状缓解时间(60.4±10.2)min,常规组总有效率72%,观察组总有效率84%。两组治疗后监测指标、症状缓解时间与总有效率比较,P<0.01,有显著性差异。结论无创正压通气操作方便,安全性较好,能够提高急性左心衰竭的救治效果,是抢救重度急性左心衰的有效手段,建议重症患者及早应用。
Objective To observe noninvasive positive pressure ventilation effect of treatment of severe acute left heart failure. Methods Intensive care units were admitted in 50 patients with severe acute left heart failure, were randomly divided into normal group and noninvasive positive pressure ventilation, to observe the heart rate, breathing rate, arterial blood gas analysis before and after the treatment： the partial pressure of oxygen partial pressure, carbon dioxide, the change of blood oxygen saturation, records of clinical symptoms. The data obtained using analysis of variance, t test and X2 test. Results Two groups before and after treatment the heart rate, breathing rate, oxygen partial pressure, co2 partial pressure, blood oxygen saturation were obviously improved, conventional group of clinical symptoms time 83.4 ± 8.6min, noninvasive positive pressure ventilation way clinical symptoms time 60.4 ± 10.2 min, conventional group total effective rate 72%, noninvasive positive pressure ventilation group total effective rate 84%. Monitoring index of the two groups after treatment, clinical symptoms time compared with the total effective rate, there are significant differences（P〈0.01）. Conclusion Noninvasive positive pressure ventilation is easy to operate, good security, can improve the treatment effect of acute left heart failure, is an effective method for severe acute left heart failure treated, suggest that in patients with severe application as early as possible.
