摘要
目的探讨不同护理分级对脑卒中住院患者直接护理时间的影响。方法脑卒中患者238例,分别按照医嘱护理分级和基于患者日常生活活动能力分级(Barthel指数),观察各个级别下患者的直接护理时间,并进行比较分析;然后将两种护理分级制度相结合,形成综合护理分级,分析综合护理分级下各个级别患者的直接护理时间,并对综合护理分级与直接住院时间之间的相关性进行分析。结果两种护理分级制度下,不同级别患者的例数分布不同(P<0.01)。医嘱护理分级下,1级护理直接护理时间高于2级与3级(P<0.01),但2级与3级比较无显著性差异;Barthel指数护理分级下,不同级别患者直接护理时间比较有显著性差异(P<0.01)。综合护理分级下,患者的直接护理时间随Barthel指数分级逐级递减;直接护理时间与护理分级赋值高度负相关(R=-0.90)。结论医嘱护理分级与Barthel指数护理分级在指导护理时间分配上各有优劣,两者相结合的综合护理分级制度更适合于临床护理。
Objective To investigate the impact of different nursing grading system on the direct nursing time in stroke patients. Methods 238 cases of patients were divided into different groups according to doctors, orders or the activity of daily living （Barthel）. Comparative analysis was done after the direct nursing time was recorded. And then, the two grading system were combined to analysis again as well as the coefficient between the direct nursing time and the given value about the grades was calculated. Results the distributions of patients in different grading system was obviously different （P〈 0. 01）. In system according to doctors, orders, the direct nursing time of grade 1 obviously longer than that of grade 2 and 3, while there was no significantly difference between the latters Meanwhile, that of all the groups in Barthel system were different （P〈0. 01）. In combined system, the direct nursing time of different Bartbel grade gradually decreased; in addition, the direct nursing time had a closely relationship with the given value about the grades （R=-0. 90）. Conclusion nursing grading system according to doctors, orders or the Barthel both had advantages and disadvantages, and the combined grading system is better in clinic.
