摘要
目的:对于全膝关节表面置换术(人工)采用中西医结合康复锻炼的方法进行治疗,观察其治疗效果。方法:选取在本院收治的骨性关节炎并进行人工全膝关节表面置换术的患者97例,随机分为观察组50例和对照组47例。对照组患者在手术后进行常规的康复锻炼,观察组在康复锻炼治疗的基础上,给予中药治疗,在手术后的第2天开始给予"舒筋活血1号汤",水煎服,2次/天,共用7天;在手术的缝合线拆除后(14天),用"舒筋活血汤2号"进行熏洗,共用30天。在术后的第2个月评定治疗效果。结果:在术后1天时比较,两组患者关节肿胀度差异不显著,P>0.05;在术后1周、2周、3周、4周时两组关节肿胀度比较,差异显著,P均<0.001;术后2个月进行随访评估,观察组患者疼痛、活动度、肌力、行走功能、总分各项评分优于对照组相对应的各项评分,差异显著,P<0.001;对照组患者出现术后感染1例,深静脉血栓3例,并发症发生率为8.51%(4/47),和观察组(并发症发生率为0.00%)比较,差异显著,P<0.05。结论:对于全膝关节表面置换术采用中西医结合康复锻炼的方法进行治疗,能够使关节肿胀消退加快,提高术后膝关节的评分,康复锻炼效果增强。
Objective： To observe the therapeutic effect of combined treatment of traditional Chinese and western medicine（ TCM） and western medicine on the treatment of total knee arthroplasty（ manual）. Methods： 97 cases of patients with osteoarthritis of the knee were selected,and were randomly divided into observation group with 50 cases and controlled group with47 cases. The controlled group was treated with conventional rehabilitation exercise. The observation group was treated with traditional Chinese medicine. After 2days of the operation,began to give the＂Shujin Huoxue Decoction No. 1＂,was decocted in water for oral dose,2times a day,lasted 7days; after the operation,the suture was removed（ 14days）. Fumigating- washing with＂Shujin Huoxue Decoction No. 2＂,lasted 30 days. Assessment of treatment efficacy in second months after surgery. Results： After 1day,there was no significant difference between the two groups of patients with joint swelling,P〈0. 05; 2weeks,3weeks,4weeks and two weeks after the 2groups of joint swelling degree,the difference was significant,P〉0. 001; and the controlled group was 1case of postoperative infection,3cases with deep venous thrombosis,the rate of complication was 8. 51%. Conclusion： For the treatment of total knee arthroplasty by using Chinese and western medicine combined with rehabilitation exercise,can make the joint swelling and regression to speed up,improve postoperative knee score,rehabilitation exercise effect.
