摘要
目的比较三种方法构建鼻咽癌患者的营养指数模型,确定各模型的最佳截断点。方法调查326例鼻咽癌患者的营养状况及生存质量并进行放射敏感性评估,采用主成分分析、独立性权数法和哑变量分析法建立三种营养指数模型;运用ROC曲线评价各模型预测患者生存质量及放射敏感性的能力并确定各模型的最佳截断点。结果在预测患者生存质量时,主成分分析法、独立性权数法和哑变量分析法构建的营养指数模型ROC曲线下面积分别为0.75、0.72和0.68,差异均无统计学意义;在预测患者放射敏感性时,三种方法的营养指数模型ROC曲线下面积分别为0.77、0.70和0.73,差异均无统计学意义。三种方法构建的营养指数模型的营养不良截断点分别为12.1、65.3和6.0。结论三种模型预测生存质量及放射敏感性的能力相同,哑变量分析法构建的营养指数模型最为简便,更适于临床推广。
Objective To compare the nutrition index model constructed by three methods in patients with nasopharyngeal carcinoma and determine optimal cut-off point of each model. Methods Nutritional status,quality of life and radiosensitivity were surveyed in 326 nasopharyngeal carcinoma patients. Principal component analysis,weight of independence and dummy variable analysis were used to establish three kinds of nutrition index model. ROC curve was used to compare each models' predictive ability of quality of life and radiosensitivity, and determine the best model diagnosis critical value. Results When predicted patients' quality of life, the area under ROC curves of principal components analysis,weight of independence and dummy variable analysis were 0. 75, 0. 72 and 0. 68, and no significant difference was found; When predicted patients' radiosensitivity,the area under ROC curves of principal components analysis,weight of independence and dummy variable analysis were 0. 77,0. 70 and 0. 73, and no significant difference was found. The optimal cut-off point of principal components analysis,weight of independence and dummy variable analysis nutrition index model were 12. 1, 65. 3 and 6. 0. Conclusion Three models have equal predictive ability of quality of life and radiosensitivity. Dummy variable analysis nutrition index model is the simplest model,which deserves to generalize on clinic.
