摘要
目的探讨骨未分化高级别多形性肉瘤(BUPS)的影像表现及病理特征。方法回顾性分析经手术病理证实的22例BUPS的影像表现。2例行X线、CT和MRI检查,5例行X线和MRI检查,1例行X线和CT检查,3例仅行CT检查,11例仅行X线检查。结果 22例中,15例发生在四肢骨,3例发生在骨盆,左肩胛骨、第6肋骨、下颌骨、第5腰椎各1例;主要表现为溶骨性骨质破坏,可合并膨胀改变,伴软组织肿块,可见骨膜反应或合并病理性骨折,病理上表现为瘤细胞的多形性,需结合免疫组织化学。结论 BUPS影像表现缺乏特征性,但结合病史,如在长骨发现溶骨性骨质破坏,伴软组织肿块应考虑该病可能,最终需依靠病理确诊。
Objective To explore the imaging findings and pathological characteristics of undifferentiated high-grade pleomorphic sarcoma of bone（ BUPS）. Methods Imaging data of 22 cases with histopathologically confirmed BUPS from January 2005 to May 2015 were reviewed retrospectively,2 cases were examined by X-ray,CT and MRI,4 cases were examined by X-ray and MRI,1 case was examined by X-ray and CT,3 cases took CT examination,and 11 cases took X-ray examination only. Results In 22 cases; including 15 male cases and 7 female cases,aged 14 to 75,among these,15 cases had the BUPS in the four limbs,3 cases in the pelvis,the rest of the cases had the tumor in the left scapula,the sixth rib,the mandible and the fifth lumbar vertebra one case for each,imaging findings include osteolytic bone destruction,some combined with local expansive changes,accompanied by soft tissue mass,periosteal reaction and pathological fracture occasionally found. By pathology tumor presented as pleomorphic cells,the combination of immunohistochemistry is needed. Conclusion The imaging findings of BUPS are not specific,however,if an osteolytic bone destruction occur in the long bones,accompanied by soft tissue mass,the diagnosis of primary undifferentiated pleomorphic sarcoma should be considered,and the final diagnosis should be made based on histopathology result.
