摘要
目的探讨彩色多普勒超声对肝硬化患者门静脉和脾静脉的血流动力学的临床意义。方法选取在2012年11月~2014年11月期间到我院就诊并入院治疗的肝硬化患者共计58例,患者按照Child-Pugh法分成三个等级,并作为观察组,再选择在同时间段体检的健康志愿者共计58例,并作为对照组。通过应用彩色多普勒超声检测所有研究对象的门静脉内径（Dpv）、门静脉血流速度（Vpv）、门静脉血流量（Qpv）、脾静脉内径（Dsv）、脾静脉血流速度（Vsv）、脾静脉血流量（Qsv）以及腹水量等各项指标,进行对比分析。结果肝硬化患者Dpv、Dsv明显高于对照组,且随肝功能分级逐渐增大（P〈0.05）;肝硬化患者Qpv、Qsv明显高于对照组,且随肝功能分级逐渐增多（P〈0.05）;肝硬化患者Vpv、Vsv明显低于对照组,且随肝功能分级逐渐减慢（P〈0.05）。结论彩色多普勒超声检测门静脉血流动力学各项指标可以有效评估肝硬化患者的病情进展程度以及是否发生门静脉高压情况,具有重要临床意义。
Objective To explore the clinical significance of color Doppler ultrasonography detection hemodynamics of portal vein and splenic vein in patients with liver cirrhosis. Methods A total of 58 cases of patients with liver cirrhosis were selected as the observation group, 58 healthy people were selected as the normal control group. Patients in both groups were given color Doppler ultrasonography detection to obtain the indexes of diameter, blood flow velocity, blood flow volume and asciticfluidof portal vein and splenic vein, the correlation between suchresults with the Child-Pugh classes were analyzed. Results Dpv and Dsvin patients with liver cirrhosis were significantly higher than those in the normal control groupandthe increase was related to the Child-Pugh（P〈0.05）; Qpv and Qsvin patients with liver cirrhosis were significantly higher than those in the normal control groupandthe increase was related to the Child-Pugh（P〈0.05）; Vsv and Vpvin patients with liver cirrhosiswere significantly lower than those in the normal controlgroupandthe decrease was related to the Child-Pugh（P〈0.05）. Conclusion The color Doppler ultrasonogr aphydetectionhemodynamics of portal vein and splenic vein can effectively determine the severity of cirrhosis,identify whether portal hypertension occurs to the cirrhosis patient, have an important significance.
