无创通气联合丹参多酚酸盐对肺心病呼吸衰竭患者血液流变学指标的影响 被引量:10

Effect of Non-invasive Ventilation Combined with Salvianolate on Haematological Indexes of Patients with Pulmonary Heart Disease Complicated by Respiratory Failure

摘要 目的探讨无创通气联合丹参多酚酸盐对肺心病呼吸衰竭患者血液流变学的影响。方法选取肺源性心脏病呼吸衰竭患者68例,随机分为对照组和试验组,各34例。对照组给予无创通气治疗,试验组在此基础上联合丹参多酚酸盐治疗。观察两组血液流变学指标及动脉血气指标变化。结果试验组总有效率91.18%高于对照组的70.59%,差异有统计学意义(P<0.05)。治疗后,两组全血高切黏度、低切黏度和血浆黏度明显降低,差异有统计学意义(P<0.05或P<0.01),其中试验组降低幅度更为明显(P<0.01)。治疗后,两组二氧化碳分压(PaCO2)、动脉血氧分压(PaO2)、动脉血氧饱和度(SaO2)均明显改善,差异有统计学意义(P<0.01),其中试验组指标变化优于对照组(P<0.01)。结论无创通气联合丹参多酚酸盐可有效改善肺源性心脏病患者的血液流变学异常,缓解血液高凝状态,促进血液循环,疗效确切,值得临床推广。 Objective To explore the effect of non-invasive ventilation combined with salvianolate on haematological indexes of patients with pulmonary heart disease complicated by respiratory failure.Methods 68 patients with pulmonary heart disease complicated by respiratory failure were selected and assigned into the control group(n=34)and the experiment group(n=34).The Patients in the control group were treated with non-invasive ventilation,while those in the experiment group were treated with non-invasive ventilation combined with salvianolate.The changes of haematological indexes and arterial blood gas(ABG)indexes were observed in two groups.Results The overall response rate of the experiment group(91.18%)was higher than that of the control group(70.59%),which suggests the significant difference between the groups(P0.05).After the treatment,the whole high and low blood viscosity as well as plasma viscosity in the two groups decreased significantly(P〈0.05 or P〈0.01),and the decrease of the experiment group was more significant(P〈0.01).Meanwhile,PaCO2,PaO2 and SaO2in the two groups all improved significantly(P〈0.01),and the improvements in the experiment group was more significant than in the control group(P 〈0.01).Conclusion Non-invasive ventilation combined with salvianolate can effectively ameliorate aberrant hemorheology of patients with pulmonary heart disease complicated by respiratory failure,relieve hypercoagulable state,and promote blood circulation.Therefore,It is of exact efficacy and worth more application in the clinical practice.
