摘要
目的探讨眶外侧锁孔入路治疗低分级（Hunt-Hess分级0～Ⅲ级）前交通动脉瘤的效果。方法回顾性分析2013年6月～2015年6月经眶外侧锁孔入路治疗28例前交通动脉瘤患者的临床资料,Hunt-Hess分级0级2例,Ⅰ级15例,Ⅱ级8例,Ⅲ级3例。术后3个月根据Glasgow预后评分（Glasgow Outcome Scale,GOS）评价治疗效果。结果术后3个月GOS 5分（恢复良好）26例,4分（轻度残疾）1例,3分（重度残疾）1例,无死亡。结论眶外侧锁孔入路作为一种微创手术入路,具有简捷、快速、微创的优点,适用于治疗低分级前交通动脉瘤。
Objective To study the clinical application and effects of treatment for low grade（ Hunt-Hess 0- Ⅲ） anterior communicating artery aneurysm（ ACo A） through lateral-orbital keyhole approach. Methods A retrospective analysis was performed on clinical data of 28 patients with low grade anterior communicating artery aneurysm admitted to our department from June 2013 to June 2015,including 2 cases of Hunt-Hess grade 0,15 case of grade Ⅰ,8 cases of grade Ⅱ,and 3 cases of grade Ⅲ,who underwent lateral-orbital keyhole approach surgery. Prognosis was evaluated according to the Glasgow Outcome Scale（ GOS） scores after 3months. Results All the 28 patients with a total of 28 ACo As were achieved successful clipping. There were 26 cases of GOS 5points（ good recovery）,1 case of GOS 4 points（ mild disability）,and 1 case of GOS 3 points（ severe disability）. No death was seen. Conclusions As a minimally invasive surgical approach,the lateral-orbital approach is a simple,rapid,and minimallyinvasive way. It is available for low-grade anterior communicating artery aneurysms.
