中性粒细胞CD64和C-反应蛋白检测在儿童感染性腹泻诊断中的意义 被引量:12

摘要 目的探讨反应蛋白检测在儿童感染性腹泻诊断中的意义。方法选择汉川市人民医院儿科细菌感染性腹泻患儿40例作为细菌感染组,病毒感染性腹泻患儿40例作为病毒感染组,非感染性腹泻患儿40例作为对照组,根据病情严重程度,将细菌感染性腹泻组分为轻度组和重度组,比较各组患儿治疗前后CD64表达情况和C-反应蛋白水平,以及C-反应蛋白和CD64对儿童腹泻的诊断情况。结果细菌感染组治疗前CD64表达及C-反应蛋白水平高于病毒感染组和对照组(P<0.05),细菌感染组治疗后CD64表达及C-反应蛋白水平和病毒感染组与对照组比较没有差异(P>0.05),细菌感染组治疗后CD64表达及C-反应蛋白水平明显低于治疗前(P<0.05),病毒感染组和对照组治疗前后CD64及C-反应蛋白水平比较没有差异(P>0.05)。治疗前轻度组的CD64表达低于重度组(P<0.05),C-反应蛋白水平和重度组比较没有差异(P>0.05);治疗后轻度细菌组腹泻患儿和重度组CD64表达和C-反应蛋白水平比较没有差异(P>0.05),轻度组和重度组治疗后CD64表达和C-反应蛋白水平均较治疗前降低(P<0.05)。CD64诊断细菌性腹泻的敏感性为87.5%,特异性为88.8%,C-反应蛋白诊断细菌性腹泻的敏感性为67.5%,特异性为76.3%,CD64诊断细菌性腹泻的敏感性高于C-反应蛋白的诊断敏感性(P<0.05)。结论 CD64和C-反应蛋白在儿童细菌感染性腹泻的诊断及预后评估中具有重要意义,CD64的表达情况能够反映儿童细菌感染性腹泻患儿的病情严重程度,对儿童细菌感染性腹泻的诊断敏感性较C-反应蛋白高。
